含少突成分胶质母细胞瘤术后治疗的卫生经济学研究

摘　　要：目的比较含少突成分胶质母细胞瘤开颅手术后应用强化治疗和非强化治疗的疗效和卫生经济学价值。方法病例数据来源于中国胶质瘤基因图谱计划(CGGA)数据库,2005年4月-2009年6月接受开颅手术,病理诊断为含少突成分胶质母细胞瘤39例。按术后治疗策略不同分为强化治疗组(n=15)和非强化治疗组(n=24)。比较两组病人术后总生存期、治疗费用等项指标。结果两组病人在年龄、性别、术前KPS评分和总生存期方面的差异无统计学意义(均P>0.05)。但在术后治疗费用方面,非强化治疗组明显低于强化治疗组,且具有统计学意义(P<0.001)。结论含少突成分胶质母细瘤术后治疗使用非强化治疗在延长病人总生存期方面与强化治疗效果相当,其费用较强化治疗低,能明显减轻病人经济负担。Objective To compare the effects and health economic value of intensive treatment and non-intensive treatment for glioblastoma with an oligodendroglioma component. Methods All the data were from Chinese Glioma Genome Atlas（CGGA）database. In total, 39 cases of patients who were diagnosed as glioblastoma with an oligodendroglioma component and received tumor resection from April 2005 to June 2009 were analyzed retrospectively. According to postoperative therapy, patients were divided into intensive treatment group（n = 15） and non-intensive treatment group（n = 24）. Overall survival and the expense of treatment were compared between the two groups. Results There was no significant difference in age, gender, preoperative Karnofsky score and overall survival between the two groups（all P〉0.05）. The expense was significantly less in non-intensive treatment group than intensive treatment group on postoperative therapy（P〈0.001）. Conclusions There is no significant difference in overall survival between intensive treatment group and non-intensive treatment group. Non-intensive therapy can significantly reduce the financial burden of the patient.
