新生儿脐静脉置管术后肝脏超声表现的临床分析

摘　　要：目的分析脐静脉置管术(UVC)后肝脏超声发现异常新生儿的影像学特征。方法对我科行UVC的新生儿术后行动态床旁肝脏彩超检查,肝脏彩超发现异常立即拔除脐静脉导管,收集临床资料及影像学资料,对症治疗并进行随诊。结果 85例新生儿中有7例肝脏彩超发现异常,其中4例考虑为肝脏脓肿,2例考虑为肝内血肿,另1例为输液渗出。随诊5例患儿预后良好,2例患儿随诊至2.5个月肝脏混合回声区较前明显缩小。结论采用UVC时需警惕肝脏并发症的发生。动态肝脏彩超监测可及早、准确发现肝脏并发症,在新生儿重症监护室有良好应用前景。Objective To analysis of abnormal liver ultrasound imaging features after umbilical venous catheterization（UVC）in neonatal.Methods Neonatal who underwent UVC were examined with dynamic bedside liver ultrasonography.The umbilical venous catheter removed immediately when abnormal liver ultrasound found abnormal.Clinical data and imaging data were collected,treated and follow-up.Results Abnormal liver ultrasonic manifestations was found in 7cases,4cases of liver abscess,2cases of intrahepatic hematoma,1case of transfusion exudative.Five cases were followed up for a good prognosis,2cases with follow-up to 2.5months mixed echogenic area significantly reduced.Conclusion Complications after UVC should be pay attention,dynamic liver ultrasound can early and accurately detect liver complications,which has a good prospect in the neonatal intensive care unit.
