结直肠癌患者部分凝血指标变化的临床意义

摘　　要：目的进一步了解结直肠癌患者部分凝血指标的变化及临床意义。方法选取我院2011年1月-2013年10月收治的结直肠癌患者132例（结直肠癌组），其中有转移的患者为A组（32例），无转移患者为B组（100例）；另选取结直肠息肉患者89例（结直肠息肉组）。采用CA7000全自动血凝分析仪测定各组血浆凝血酶原时间（盯）、活化部分凝血活酶时间（APTT）、凝血酶时间（TT）、纤维蛋白原（FIB）和D-二聚体（D—D）。结果结直肠癌组患者血浆PT、TT、FIB、D—D水平较结直肠息肉组有明显增高，差异有统计学意义（P〈0．05），有转移患者与无转移或侵犯患者的TT、FIB、D—D明显增高，且差异有统计学意义（P〈0．05），有转移患者与无转移患者的PT、APTT比较有增高倾向，但差异无统计学意义（P〉0．05）。结论结直肠癌患者血浆FIB与D—D水平增高，提爪患者体内血液处于高凝状态，而PT及TT时间延长提示结直肠癌患者可能有出血倾向，表明凝血功能异常与肿瘤的发生、发展密切相关。Objective To explore the changes of blood coagulation parameters in patients with colorectal cancer and their clinical significance. Methods One hundred and thirty-two patients with colo- rectal cancer（ colorectal cancer group） were recruited in Renmin Hospital of Wuhan University from Jan. 2011 to Oct. 2013 ,they were divided into 3 groups;32 patients with metastasis （Group A） ;100 patients without metastasis（ Group B） and 89 patients with colorectal polyps （Group C ）. CA7000 automatic blood coagulation analyzer was used to determinethe levels of prothrombin time（ PT）, activated partial thrombo- plastin time（APTT） ,thrombin time（TT） ,fibrinogen（FIB） and D-dimer（D-D）. Results The levels of PT,TT, FIB, D-D in patients with colorectal cancer were higher than those in the patients with colorectal polyps, and the difterences were significant （ P 〈 0.05 ） ; 3T, FIB, D-D increased significantly in the pa- l ien/s with metastasis than those without metastasis（P 〈 0.05 ）. PT,AtYFT in patients with metastasis ten- （ted to be higher than those without metastasis, but the differences were not significant（ P 〉 0.05 ）. Con- clusion Patients with colorectal cancer have higher plasma levels of FIB and D-D, presenting a high co- agulation state;as well as extension of PT and TT time,indicating bleeding tendency.
