骨痛灵方联合西药治疗恶性肿瘤骨转移疼痛34例临床观察

摘　　要：目的观察骨痛灵方联合唑来膦酸治疗恶性肿瘤骨转移疼痛的临床疗效。方法将67例恶性肿瘤骨转移符合疼痛评定标准Ⅱ～Ⅲ级疼痛患者随机分为治疗组34例和对照组33例。基础治疗：根据三阶梯止痛药选择方法给予两组骨转移疼痛患者止痛药治疗。对照组每4周静脉滴注唑来膦酸1次，每次4mg加入0．9％氯化钠100ml，静脉滴注，治疗组在对照组的基础上服用骨痛灵方，每日1剂。两组疗程均为8周，治疗前后记录患者疼痛评分（VAS评分）和生活质量评分（KPS评分），判定疗效。结果两组治疗后VAS评分均明显下降（P〈0．05），治疗后治疗组VAS评分低于对照组（P〈0．05）。治疗组治疗后KPS评分提高明显（P〈0．05），对照组治疗后KPS评分改善不明显（P〉0．05）；治疗后治疗组KPS评分较对照组显著提高（P〈0．05）。治疗组疼痛有效率（64．71％）及生活质量有效率（64．71％）明显高于对照组（36．36％及30．30％），差异均有统计学意义（P〈0．05或P〈0．01）。结论骨痛灵方联合唑来膦酸与单纯使用唑来膦酸相比较，镇痛效果明确，能明显改善患者生存质量。Objective To observe the therapeutic effect of Gutongling Formula combined with zoledronic acid on malignancy bone metastasis pain. Methods Totally of 67 selected patients of malignancy bone metastasis with a grade II or grade III pain according to WHO standards were randomly assigned to a control group of 33 cases and a treatment group of 34 eases. Basic treatment： analgesic treatment according to three step analgesic ladder was applied to both groups. The control group was treated with intravenous drip of zoledronic acid 4 mg into normal saline once every 4 weeks, while the treatment group was added with Gutongling Formula once dose per day. The treatment course was 8 weeks. Visual analogue scale （VAS） scores arid karnofsky （KPS） scores were recorded before and after treatments to evaluate the effect. Results After treatment, the VAS of both groups had a significant reduction （ P 〈 0.05 ） , VAS of the treatment group was lower than that of the control group （ P 〈 0. 05 ）. The KIPS scores of the treatment group were significantly improved after treatment （P 〈 0.05） , while there was no obvious improvement in the control group （P 〉 0. 05） and the difference of KPS scores of the treatment group was statistically significant as compared with that of the control group （P 〈 0. 05）. The effective rate of pain relieve and quality of life of the treat- ment group was 64.71% and 64.71% respectively, which was notely higher than 36. 36% and 30.30% of the control group （P 〈0. 05 or P 〈0. 01 ）. Conclusion Tire combined treatment of Gutongling Formula and zoledronie acid had an obvious analgesic effect and can significantly improve the quality of life of patients comparing with zoledronic acid treatment alone.
