摘  要:目的探讨基质金属蛋白酶(MMP-9)基因R279Q位点多态性与脑梗死初发及复发的相关性。方法选择2012年4月—2014年4月在遂宁市中心医院神经内科住院的初发脑梗死患者350例作为初发脑梗死组,复发脑梗死患者340例作为复发脑梗死组,同期在我院体检健康者360例作为对照组,采用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)分析法检测MMP-9基因R279Q位点多态性,酶联免疫吸附法(ELISA)检测血清MMP-9水平。结果初发脑梗死组、复发脑梗死组MMP-9基因R279Q位点GG基因型频率及G等位基因频率高于对照组,复发脑梗死组高于初发脑梗死组(P<0.05)。3组受试者性别、年龄、体质指数、糖尿病发生率、饮酒率、总胆固醇、三酰甘油、HDL-C、空腹血糖比较,差异均无统计学意义(P>0.05);3组受试者高血压发生率、吸烟率、LDL-C、MMP-9比较,差异均有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,血清MMP-9水平增高是脑梗死初发的独立危险因素〔OR=1.336,95%CI(1.031,1.072),P=0.028〕,但不是脑梗死复发的独立危险因素〔OR=0.609,95%CI(0.428,1.006),P>0.05〕;MMP-9基因R279Q位点多态性是脑梗死初发〔OR=1.496,95%CI(1.028,2.159),P=0.007〕和脑梗死复发〔OR=1.625,95%CI(1.072,2.566),P=0.005〕的独立危险因素。结论 MMP-9基因R279Q位点多态性是脑梗死初发和脑梗死复发的独立危险因素,MMP-9基因可能是一种潜在的脑梗死遗传易感基因。Objective To investigate the correlation between matrix metalloproteinase -9 (MMP- 9 ) gene R279Q polymorphism and first onset and recurrence of cerebral infarction. Methods From April 2012 to April 2014 in the Department of Neurology, Suining Central Hospital, 350 first onset cerebral infarction patients were selected as A group, 340 recurrent cerebral infarction patients were selected as B group, and 360 healthy cases were seled as control group. Polymerase chain reaction - restriction fragment length polymorphism ( PCR - RFLP) method was used to analyze the MMP - 9 gene P6279Q polymorphism, enzyme linked immunosorbent assay (ELISA) was used to detect the serum MMP- 9 level. Results GG genotype frequency and G allele frequency of groups A and B were higher than those of control group, respectively (P 〈0. 05 ), and those of A group were higher than those of B group ( P 〈 0. 05 ). No significant differences of gender, age, BMI, incidence of diabetes, drinking rate, TC, TG, HDL - C, fasting blood glucose among the three groups ( P 〉 0.05 ) ; while there was significant differences of incidence of hypertension, smoking rate, LDL - C, MMP - 9 among the three groups ( P 〈 0. 05 ). Multiple Logistic regression analysis showed that, higher serum MMP - 9 level was an independent risk factor of first onset of cerebral infarction [ OR = 1. 336, 95% CI ( 1. 031, 1. 072) , P = 0. 028 ] , but was not of recurrence of cerebral infarction [ OR =0. 609, 95% CI (0. 428, 1. 006) , P 〉0.05 ] ; MMP - 9 gene R279Q polymorphism was an independent risk factor of first onset of cerebral infarction [ OR = 1. 496, 95% CI ( 1. 028, 2. 159), P = 0. 007 ] and recurrence of cerebral infarction [OR=1.625, 95%C/ (1.072, 2.566), P=0.005], respectively. Conclusion MMP-ggeneR279Qpolymorphismisan independent risk factor of first onset and recurrence of cerebral infarction, MMP- 9 gene may be a potential genetic susceptibility gene of cerebral infarction.
