霍奇金淋巴瘤斗篷野等中心和延长源皮距放疗的剂量学比较

摘　　要：目的对Ⅰ、Ⅱ期霍奇金淋巴瘤斗篷野常规放疗采用等中心和延长源皮距治疗进行剂量学比较,评估不同照射方法的优缺点。方法选择10例病理证实的Ⅰ、Ⅱ期淋巴瘤患者行CT定位,用Philip Pinnacle治疗计划系统对每例患者分别作出等中心和延长源皮距2种不同布野的常规计划,分析各治疗计划的PTV剂量分布均匀性及心脏、肺、脊髓的受量。结果斗篷野内不同部位照射剂量差别在20%-30%。腋窝和颈部淋巴深度浅,接受相对高的剂量,而纵膈相对较低。剂量分布均匀度与适形度无明显差别。结论斗篷野采用延长SSD照射较SAD能降低心、肺、脊髓的剂量,但剂量分布均匀度与适形度无明显差别。Objective To compare the dose distribution of mantle-field radiotherapy using SAD Radiotherapy and lengthen SSD radiotherapy techniques in stage I and Ⅱ Hodgkins lymphom（HL）.Methods Ten patients with patholocally proved early stage HL underwent CT simulation,then both SAD and lengthen SSD planning performed using Pinnacle treatment planning system（TPS）.The dosimetric parameters of different irradiation plans were analyzed,including Conformal index（CI）,homogeneity index（HI）.Results The different location irrational dose between 20%-30% in mantle-field,armpit and neck lymph have a shallow depth,they accept the higher dose relative mediastinum,The isodose distribution and homogeneity of PTV were similar.Conclusions mantle-field radiotherapy using lengthen SSD Radiotherapy compared SAD Radiotherapy can reduce the dose of heart,lung and spinal cord.But the isodose distribution and homogeneity of PTV were similar among the two techniques.
