摘 要:目的分析河南地区不同孕期孕妇甲状腺功能,建立本地区健康孕妇妊娠各期甲状腺功能的参考范围。方法采用化学发光分析方法检测107例健康妇女和2 696例健康孕妇血清总甲状腺素(TT4)、总三碘甲状腺原氨酸(TT3)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)和促甲状腺激素(TSH)浓度;统计分析,建立妊娠各期甲状腺激素水平参考范围,用中位数(M)及双侧限值[P2.5,P97.5]表示。结果孕期各组之间及与孕前组比较TT4、FT4和TSH水平之间差异均有统计学意义(P<0.01);孕早期、中期、晚期TSH激素水平均低于孕前组,随着孕周的增加而升高,孕早期最低,约为孕前TSH水平的61.8%;FT3和FT4在妊娠各期均低于孕前组,FT4水平在妊娠各期均低于孕前组,并在妊娠期间随着孕周的增加而降低,至孕晚期,FT4水平为孕前期水平的64.4%;TT3、TT4在孕中期达到最高,孕期TSH和FT4相比其他甲状腺激素指标在孕期变化更明显。结论孕妇妊娠期甲状腺激素水平发生特异性变化,与非妊娠妇女甲状腺激素水平比较差异有统计学意义,各孕期之间比较差异有统计学意义。Objective To analyze thethyroid function of healthy pregnant women in different gestation period from Henan and establish reference interval of thyroid of them. Methods A total of 107 healthy women and 2 696 healthy pregnant women were tested for serum thyroid( TT4,TT3,FT3,FT4,TSH) using automated chemiluminescence immunoassay. To analyze the different measurements using statistical methods. To establish the specific reference ranges of thyroid hormones in different gestation period. The reference range of each trimester hormone was calculated as median( M value) and two- sided limits( the 2. 5 th and 97. 5th percentile values). Results There were statistical significance in the difference of TT4,FT4 and TSH between every gestation period group and non- pregnant group( P〈 0. 01); The TSH level in early,middle and late pregnancy groups were all lower than in non- pregnant group,and to the minimum in the early pregnancy,just about 61. 8% of the non- pregnant,but raised with the increase of gestational age; The FT4 levels in pregnancy were also lower than non- pregnant,but reduced with the increase of gestational age,there was only 64. 4% of non- pregnant levels in late pregnancy. In addition,the TT3,TT4 level reached the highest in the middle of pregnancy. The FT4 and TSH changes were more obvious compared with other thyroid hormone in all gestation period. Conclusion The thyroid hormone level was significantly different for pregnancy,and compared with non- pregnant women,the difference had statistical significance,and the differences among all gestation periods were also statistically significant.
