摘  要:目的采用表面增强激光解析电离飞行时间质谱(SELDI-TOF-MS)技术检测壶腹癌患者血清蛋白指纹图,并建立诊断模型。方法用Ciphergen公司生产的PBSII/c型蛋白质指纹图谱仪和Protien Chip CM10蛋白芯片采集27例壶腹癌、30例对照组(良性胆道梗阻患者)血清蛋白质谱图,采用Biomaker Wizard 3.1版分析软件筛选差异蛋白。结果发现壶腹癌患者与对照组血清蛋白质指纹图谱有32个差异表达的蛋白质(P<0.05),其中M/Z 6 583.93敏感性为74.1%,特异性为63.3%,受试者工作特征曲线下面积(AUC)=0.706;M/Z 8 612.67敏感性为77.8%,特异性为80.0%,AUC=0.787。以此建立的壶腹癌的诊断模型敏感性为88.9%,特异性为83.3%,AUC=0.86。通过蛋白质数据库检索M/Z 6 583和M/Z 8612分别得到2和4个候选蛋白。结论利用SEL-DI-TOF-MS技术能发现壶腹癌患者血清中的特征蛋白,对壶腹癌的早期诊断具有重要意义。【Objective】To develop a novel and reliable method and establish diagnostic model for diagnosis of the ampullary carcinoma by surface enhanced laser desorption/ionization time-of-flight mass spectrometry(SELDITOF-MS). 【Methods】The study included 27 cases with ampullary carcinoma, 30 cases of the control group(patients with benign biliary obstruction). For serum protein signature analysis, the SELDI-TOF-MS analysis was performed to screen the serum protein markers and the CM10 protein chips were used for analysis. 【Results】The data analysis revealed 32 differential expression peaks(P〈0.05). M/Z 6 583.93 set, it yielded a sensitivity of 74.1%, a specificity of 63.3%, area under the ROC(AUC)=0.706; M/Z 8 612.67 set, it yielded a sensitivity of 77.8%, a specificity of 80%, AUC=0.787. A panel of two biomarkers was selected to set up a diagnostic model for diagnosis of the ampullary carcinoma. The sensitivity and specificity were 88.9% and 83.3% respectively, AUC=0.86. The M/Z 6 583.93 and M/Z 8 612.67 were retrieved through the protein database and obtained two kinds of candidate proteins and four kinds of candidate proteins, respectively. 【Conclusions】SELDI-TOF-MS may provide a new proteomics mode for laboratory diagnosis of ampullary carcinoma. These specific biomarkers are potential tools for early diagnosis and identification of ampullary carcinoma.
