头颈姿势与矢状向颅面发育的相关性研究

摘　　要：目的通过研究不同矢状骨面型患者自然头位头颈姿势,分析矢状向颅面发育与头颈姿势的相关性。方法选取发育高峰前期和高峰后期骨性I类、II类、III类错牙合患者自然头位头颅定位侧位片各30张,测量头位变量SN-VEL、SN-OPT、SN-CVT、OPT-CVT、CVT-HOR,颅面结构变量SNA、SNB、ANB、SN-GoGn,比较同一发育时期不同矢状骨面型错牙合患者头颈姿势的差异。结果发育高峰前期,SN-VEL、SN-CP、OPT-CVT在I类、II类、III类各组间的差异无统计学意义(P>0.05);SN-OPT、SN-CVT、CVT-HOR,在I类、II类、III类各组间差异有统计学意义(P<0.05)。发育高峰后期,SN-VEL、SN-CP、SN-OPT、SN-CVT、OPT-CVT、CVT-HOR在I类、II类、III类各组间差异有统计学意义(P<0.05)。结论在发育高峰前后期不同矢状骨面型患者的头颈姿势存在一定差异。Objective To study the characteristics and differences of patients＇cervical vertebrae posture with different facial types to predict the sagittal growth of the patients. Methods 30 cephalometric radiograph of the patients with class I, class II, class III malocclusion which in their growth peak period and 30 in their later peak period from the orthodontic department of the stomatology hospi- tal were included to measure and analyze the variance of the head position ： SN - VEL, SN - OPT, SN - CVT, OPT - CVT, CVT - HOR,and the variance of the cranial - facial structures ： SNA, SNB, ANB, SN - GoGn. The differences of the cervical vertebrae postures of pa- tients with different sagittal skeletal patterns malocclusiond were compared in the same development period. Results Before the growth spurts ： the indexes of SN - VEL, SN - CP, OPT - CVT in the class I, class II, class III among the groups were not statistically significant （P 〉 0.05） ; the index of SN - OPT, SN - CVT, CVT - HOR in the class I, class II, class III among the groups were statistically signifi- cant （ P 〈 0.05 ）. After the growth spurts ： the indicators of SN - VEL,SN - CP, SN - OPT, SN - CVT, OPT - CVT, CVT - HOR in the class I, class II, class III among the groups were statistically significant （ P 〈 0.05 ）. Conclusion There are differences before and after growth spurts with different sagittal skeletal pattern in cervical vertebrae posture.
