摘　　要：目的探讨高尔基体蛋白73(GP73)和甲胎蛋白(AFP)在肝癌中的诊断价值。方法本实验共收集199例血清,其中肝恶性肿瘤患者83例,肝硬化患者40例,肝炎患者36例,健康体检者40例。采用酶联免疫法检测血清样品中GP73的浓度,应用电化学发光免疫法测定血清标本中AFP的含量。结果 GP73临界值取112.5 ng/ml时,诊断HCC的灵敏度和特异度达到最佳,分别为83.13%和77.58%,ROC曲线下面积(area under curve,AUC)为0.825;AFP临界值取10 ng/ml时,诊断HCC的灵敏度和特异度分别为75.90%和70.69%,AUC为0.756。两者联合检测时敏感性可达91.57%。结论 GP73可作为一项较好的肝癌诊断指标,与AFP联合检测,可提高肝癌诊断的敏感性。Objective To investigate the value of serum golgi protein 73（GP73） in the diagnosis of hepatocellular carcinoma（HCC）.Methods One hundred and ninety-nine serum samples were collected from 83 cases of HCC,40 cases of hepatic cirrhosis,36 cases of hepatitis and 40 cases of healthy people.ELISA was used to detect the GP73 concentrations in the 199 serum samples,and the electrochemiluminescence immunoassay was used for determining the contents of AFP in these serum samples.Results The critical value of GP73 was 112.5 ng/ml,the optimal sensitivity and specificity of GP73 in the diagnosis of HCC were 83.13% and 77.58%,and the area under the ROC curve was 0.825.While the critical value of AFP was10.0 ng/ml,the best sensitivity and specificity in diagnosis of HCC were 75.90% and 70.69%,respectively,and the AUC was 0.756.The sensitivity of combined diagnosis can reach 91.57%.Conclusion GP73 is a better biomarker in diagnosis of HCC,and combined detection of GP73 and AFP may improve the diagnostic efficiency for HCC.
