高血压脑出血急性期血压控制与血肿大小的关系

摘　　要：目的探讨高血压脑出血急性期血压与血肿大小的关系。方法高血压脑出血患者83例,入院时均进行CT检查确诊。根据患者入院时的收缩压将其分为三组即I组:140 mm Hg≤SBP≤159mm Hg(1 mm Hg=0.133 kPa);Ⅱ组:160 mm Hg≤SBP≤179 mm Hg;Ⅲ组:SBP≥180 mm Hg。根据情况给予镇静、脱水、降颅压治疗,并动态监测血压,必要时给予药物降压治疗。根据收缩压控制情况为两组:A组:SBP<160 mm Hg;B组:SBP≥160 mm Hg。观察血压控制与血肿大小。结果高血压脑出血后血压均升高,特别是发病6 h内血压升高明显。对三组间血肿扩大率进行分析,差异有统计学意义(P<0.05)。收缩压控制情况与血肿扩大关系:A组(SBP<160 mm Hg)血肿扩大率18.60%,B组(SBP≥160 mm Hg)血肿扩大率70.80%。两组比较,差异有统计学意义(P<0.05)。结论高血压脑出血患者急性期适度血压控制程度与血肿扩大有一定的相关性。Objective To study the correlation between blood pressure（BP） control and hematoma size for acute hypertensive cerebral hemorrhage patients. Methods Selected 83 patients with hypertensive cerebral hemorrhage, and divided them into three groups according to systolic blood pressure, group I： 140 mm Hg≤SBP ≤ 159 mm Hg; （1 mm Hg=0.133 kPa） group Ⅱ ：160 mm Hg ≤ SBP ≤ 179 mm Hg; group Ⅲ ： SBP ≥180 mm Hg. Under the circumstances, given sedation, dehydration, lower intracranial pressure treatment, or antihypertensive medication treatment according to conditions and dynamic monitored blood pressure of patients. According to regulation of systolic blood pressure, patients were divided into two groups; group A： SBP〈160 mmHg, group B： SBP ≥ 160 mm Hg, relationships between the blood pressure regulation and hematoma enlargement during acute stage of hypertensive cerebral hemorrhage were observed. Results Blood pressure of patients was higher, especially within 6 hours. Compared extension ratio of three groups, the difference was statistical significance（P〈0.05）. The relationship of regulation of blood pressure and blood tumor enlargement： group A （SBP〈160 mm Hg）, the ratio of extension was 15.60%; group B（SBP ≥ 160 mm Hg）, the ratio of extension was 70.80%. The difference was statictical significance（P〈0.05）. Conclusion The control of the blood pressure has certain correlation with hematoma expansion.
