腹腔镜下阔韧带子宫肌瘤剔除术35例临床体会

摘　　要：目的：观察腹腔镜手术治疗阔韧带子宫肌瘤的临床疗效。方法35例行腹腔镜下阔韧带子宫肌瘤剔除术患者。其中子宫肌瘤直径3～12 cm,平均直径7.2 cm。所有患者均采用全身麻醉,已婚者术前放置举宫器,术中用垂体后叶素减少出血量,术后盆腔内放置引流管。结果35例均成功在腹腔镜下完成手术,无术中并发症发生。手术时间45～135 min,平均时间70.5 min。出血量50～220 ml,平均出血量90 ml。肛门排气时间8～30 h,平均排气时间16.5 h。术后住院时间3～6 d,平均住院时间4 d。结论阔韧带子宫肌瘤行腹腔镜下剔除术安全可行,但应掌握腹腔镜下各种手术技巧,这是手术成功的关键。Objective To investigate the clinical effect of laparoscopic treatment in 35 cases of broad ligament myoma.MethodsRetrospectively analyzed the clinical data of 35 patients with broad ligament myoma. The mean diameter of the tumors was 7.2 cm（3～12 cm）. Under general anesthesia uterus-lifting apparatus was employed in non-virgin patients.Pitnitrin was used to reduce intraoperative blood loss. drainage tube was placed after the procedure.ResultsThe procedure was completed successfully in all patients without injuries to the ureter and adjaeent organs. The mean operation time was 68.8 min（50～115 min）. And the blood loss ranged 30～200 ml with an average of 80 ml. The gastrointestinal function recovered in 6～28 h （average 16.5 h） after the operation. The patients were discharged from hospital within a mean of 4 postoperative days（3～6 d）. ConclusionLaparoscopie myomectomy is safe and effective for broad ligament myoma.However, a variety of laparoscopic surgical techniques are the key to successful operation.
