肺结核合并糖尿病90例临床研究

摘　　要：目的探讨和总结肺结核病合并糖尿病的临床特点及经验,为科学防治提供依据。方法回顾性分了2009年1月—2013年12月5年间来我中心门诊就诊的90例肺结核病人资料,选取45例肺结核合并糖尿病患者作为研究组,45例同期单纯性肺结核患者作为观察组。两组都用抗结核固定剂量复合剂治疗,研究组积极采用控制血糖的措施,两组其他辅助治疗手段一致,治疗期结束后对照分析临床效果。结果两组患者经治疗6个月后在临床情况中,一般临床症状、X线提示及痰菌转阴改善情况,观察组分别为40(88.9%)、35(77.8%)及40(88.9%),研究组分别为39(86.6%)、17(37.8%)及28(40.0%),且观察组治愈23例,好转17例,未愈5例,总有效率达88.9%,研究组治愈18例,好转10例,未愈17例,有效率为62.2%。经治疗9-12个月后,观察组总有效率达97.8%,研究组总有效率达95.6%,由此可知,与单纯性肺结核患者相比,肺结核病合并糖尿病患者病情重,治疗效果较差,差异有统计学意义(P<0.05)。结论肺结核病合并糖尿病应该严格控制血糖水平,在此基础上进行早期规律联合的抗痨治疗,同时必须根据患者的治疗情况适当延长治疗的时间,有助于病情的及时控制。Objective To explore and summarize the clinical characteristics of pulmonary tuberculosis complicated with diabetes and experience, to provide the scientific basis for prevention and control. Methods A retrospective analysis of 90 cases of pulmonary tuberculosis patient data in 2009 January to 2013 year in December to our center clinic,we selected？45 cases of pulmonary tuberculosis with diabetes patients as study group, 45 cases in patients with pulmonary tuberculosis as the observation group. All two groups with Fixed-does Combination drug treatment, study group actively adopt measures of control blood sugar, two sets of other auxiliary treatment means, after the end of the treatment period control analysis of clinical effect.Results Two groups of patients after 6 months of therapy in the clinical situation, the general clinical symptoms, X-ray and sputum negative conversion to improve the situation, the observation group were 40（88.9%）, 35（77.8%） and 40（88.9%）, the study group were 39（86.6%）, 17（37.8%） and 28（40%） the observation group, and 23 cases were cured, 17 cases improved, 5 cases ineffective, the total effective rate was 88.9%, the study group 18 cases were cured, 10 cases improved, 17 cases ineffective, efficiency is 62.2%. After 9-12months after treatment, the observation group the total effective rate was 97.8%, the total effective rate of 95.6%, therefore, compared with the simple pulmonary tuberculosis, pulmonary tuberculosis patients complicated with diabetes severity, treatment effect is poor, the difference was statistically significant（P〈0. 05）. Conclusion Diabetes complicated with pulmonary tuberculosis should be strict control of blood glucose level, on the basis of anti tuberculosis therapy combined with early rules, at the same time must be appropriate to extend the duration of treatment according to the patient＇s treatment, timely control and contribute to disease.
