幽门螺杆菌感染与食管癌的相关性研究

摘　　要：目的探讨幽门螺杆菌(HP)感染与食管癌发生的关系。方法将我院2008—2010年间住院手术的48例食管鳞癌患者作为观察对象,同时选取食管重度不典型增生患者40例,食管轻-中度不典型增生患者52例,慢性食管炎患者50例以及正常食管黏膜者50例作为对照对象,采用快速尿素酶法及Warthin-Starry银染两种方法检测HP感染。结果食管癌组、重度不典型增生组、轻-中度不典型增生组、慢性食管炎和正常食管黏膜组的HP感染阳性率依次降低,食管癌组与重度不典型增生组、轻-中度不典型增生组、慢性食管炎和正常食管黏膜组比较,均有统计学差异(P<0.05)。HP感染与食管癌患者的性别、年龄、部位、肿瘤大小以及有无淋巴结转移等因素无关(P>0.05),与分化程度有关(P<0.05)。结论食管中HP的感染可能参与了食管癌的发生、发展过程。Objective To explore the relationship between helicobacter pylori（ HP） infection and esophageal cancer. Methods Patients admitted to our hospital from 2008 through 2010 were divided into esophageal squamous cell carcinoma group（ n = 48）,severe esophageal dysplasia group（ n = 40）,mild-moderate esophageal dysplasia group（ n = 52） and chronic esophagitis group（ n =50）,and subjects with normal esophageal mucosa were enrolled in normal subject group（ n = 50）. The positive rates of HP were compared among the groups. HP infection was detected by rapid urease test with Warthin-Starry staining. Results The positive rates of HP decreased in sequence in esophageal squamous cell carcinoma group,severe esophageal dysplasia group,mild-moderate esophageal dysplasia group,chronic esophagitis group and normal subject group. The detection rate of HP was higher in esophageal squamous cell carcinoma group than in any of other groups（ P〈0. 05）. The detection rate of HP in esophageal squamous cell carcinoma was not associated with age,gender,location,tumor size,lymph node metastasis（ P〉0. 05）,but associated with differentiation of esophageal carcinoma（ P〈0. 05）. Conclusion It is suggested that HP infection in esophagus is associated with the development of esophageal cancer.
