摘　　要：目的探讨眼附属器黏膜相关边缘区B细胞性（MALT）淋巴瘤的临床病理学特征。方法回顾性分析74例眼附属器MALT淋巴瘤的临床资料和临床病理资料。结果该组眼附属器MALT淋巴瘤好发于中老年男性,其中男性48例,女性26例,男女之比1.85∶1。年龄22-82岁,平均年龄59.6岁;〉50岁者62例。最常见的原发部位为眶内球外附属脂肪结缔组织。组织学上该部位的MALT淋巴瘤有其不同于其他解剖部位该肿瘤的特点：弥漫性病变模式最常见,可见滤泡残留、脂肪累及、肌肉累及、泪腺残留和肿瘤细胞滤泡植入。肿瘤细胞主要为边缘带细胞,可见单核细胞样和浆细胞样分化;淋巴上皮病变不常见。肿瘤细胞B细胞表面抗原和bcl-2多弥漫（＋）,T细胞表面抗原多为（-）;CD5、CD10、cyclin D1和EBER通常为（-）,Ki-67较低。结论眼附属器MALT淋巴瘤是眼科临床病理中一类比较常见的疾病,其在发病年龄、性别和组织学表现上都具有明显的临床病理特征,掌握这些对临床病理诊断将有较大的帮助。Objective To study the clinicopathologic characteristics of MALT lymphoma in orbital and ocular adnexal tissues. Methods Clinical manifestations and the characteristics of clinical pathology of seventy-four MALT lymphoma patients in orbital and ocular adnexa were analyzed retrospectively. Results In this series of MALT lymphoma, the patients were mostly older adults, with a male preponderance. The primary site was orbit （46 cases, 62. 1% ）. The MALT lymphoma of this site had different trait from that at other sites. The most common pathological pattern was diffuse change. Reactive follicles, fatty infiltration, muscular infiltration, lacrimal gland and follicle implantation were present. MALT lymphoma was characterized by the expansion of a heterogeneous cell population, consisting of centroeyte-likc, monocytoid, and plasmacytoid cells, with occasional blasts in the marginal zone surrounding reactive follicles. Well-formed lymphoepithelial lesions were only eneounted occasionally among those cases involving the lacrimal gland. Marginal zone lymphoma cells were typically B-cell antigen^＋ , T-cell antigen^- , and CD5^- , CD10^- , CYCLIN D1^ - , and EBV^-. Conclusion MALT lymphoma in orbital and ocular adncxa is common among patients. There is characteristic in age, gender and histologic feature, which is helpful to understand these characteristics for pathologic diagnosis.
