河南省小儿脑瘫流行病学现况调查及分析

摘　　要：目的调查河南省0～6岁儿童脑性瘫痪（CP）流行特征及康复干预现状。方法于2011年9月至2012年9月期间，根据河南省各地区儿童出生率采用分层整群随机抽样原则，抽取9个市共24个区乡镇0～6岁全体常住儿童作为调查对象，以问卷形式进行入户流行病学调查，采用Epidata3．2建立数据库，选用SPSS17．0版软件进行统计学分析。结果共实际调查0～6岁儿童51 108例，发现脑瘫患儿120例，患病率为2．35‰；CP分型以痉挛型为主（占63．33％）；脑瘫患儿在母亲孕周、出生体重、出生时头围、身长、父母年龄方面与正常对照儿童问差异均具有统计学意义（P〈0．05）。脑瘫患儿中有50．0％选择医院集中式康复，享受医保或新型农村合作医疗部分报销的患儿比例为69．17％，获得政府专项经费资助者占9．17％。结论河南省0～6岁儿童脑瘫患病率为2．35‰，CP分型以痉挛型为主，患儿康复干预现状亟待加强。Objective To investigate the epidemiological characteristics of and rehabilitation interventions for cerebral palsy （CP） among children up to 6 years old in 1-Ienan Province. Methods Children from 24 towns were selected using stratified cluster sampling based on the birth rates of the various regions of the province. A house- hold questionnaire survey was conducted from September, 2011 to September, 2012. Results Among 51,108 chil- dren aged 0-6 years surveyed, 120 were found to have CP, a prevalence of 2.35‰. Most （63.33%） had spastic CP. Compared with normal children, the CP children were on average significantly different in gestational age, birth weight, birth head circumference and birth body length, in addition to their parents＇ ages. Only 50% of the children had received intensive rehabilitation training in a hospital, with 69% of those getting reimbursement from health in- surance or the new Rural Co-operative Medical Care System and insurance. Eleven （9%） said they were receiving special economic assistance from the government. Conclusion The prevalence of cerebral palsy among children aged 0 to 6 years is 2.35‰ in Henan Province, mainly of the spastic type. Rehabilitation for such children needs to be strengthened.
