摘　　要：目的：探讨3.0T MR肝脏加速容积采集（LAVA）技术联合扩散加权成像（DWI）对肝硬化背景上小肝癌的检出与定性的价值。方法回顾性分析小肝癌患者的 MRI 资料，均行 LAVA 序列多期增强扫描及 DWI（b 值800 s/ mm^2）。分析比较 LAVA增强（A 组）、DWI（B 组）、二者联合（C 组）对小肝癌的检出率；LAVA 增强、LAVA 增强联合 DWI[高信号＋病灶表观扩散系数（ADC）值＋病灶与周围肝组织 ADC 值的比值]在小肝癌定性方面的价值。结果LAVA 增强联合 DWI 较单独使用 LAVA 增强检出小肝癌高，但两者之间无统计学差异。LAVA 增强联合 DWI 与单独使用 LAVA 增强在小肝癌定性方面比较有统计学差异。结论LAVA 技术联合 DWI 可弥补单独使用 LAVA 技术对肝硬化背景上部分小肝癌检出的不足；LAVA 技术联合 DWI 可以为肝硬化背景上小肝癌的定性提供更多有价值的信息。Objective To explore the value of combination of 3.0T MR LAVA technology and DWI in detection and qualitative diagnosis of small hepatocellular carcinoma （SHCC）on the background of liver cirrhosis.Methods MRI data of patients with SHCC were retrospectively analysed.LAVA sequence enhanced multi-phase scanning and DWI （b value of 800 s/mm^2 ）were performed. The detection rate of SHCC was comparatively analysed among LAVA enhanced group （group A）、DWI group （group B）and their combination （group C）.The qualitative diagnosis value for SHCC between LAVA enhanced and the combination of LAVA enhanced and DWI （high signal＋lesions of ADC value＋lesion-to-liver ADC ratio）was also evaluated.Results The SHCC detection rate of group C was higher thangroup A,but there was no significant difference between the two groups.There was statistically significant difference between group Aand group C in the qualitative diagnosis value for SHCC.Conclusion The combination of LAVA tech-nique and DWI can make up the deficiency of LAVA technique for the detection of SHCC on the background of liver cirrhosis,and it can provide more valuable information for the qualitative diagnosis of SHCC on the background of liver cirrhosis.
