小细胞肺癌的CT征象特点分析

摘　　要：目的分析小细胞肺癌的CT征象特点及病理表现。方法回顾性分析34例经手术病理证实的小细胞肺癌的CT增强表现,重点分析小细胞肺癌累及支气管、血管、淋巴组织及胸膜的表现。结果 34例中,各级支气管受累29例,主要表现为支气管受压、变窄,支气管阻塞发生较晚;血管受累及24例,表现为血管受侵受压、变细,呈冰冻状,当肿块与肺门、纵膈的肿大淋巴结融合时,形成"冰冻纵膈";淋巴组织受累及34例,表现为自肿块向周围肺野放射状排列的线状阴影、间隔线、颗粒状阴影,纵膈、肺门及胸外淋巴结转移;胸膜受累17例,表现为胸腔积液12例(双侧6例),胸膜增厚5例。结论通过对小细胞肺癌受累及支气管、血管、淋巴组织及胸膜的分析,对于早期明确小细胞肺癌性质及分期有重要临床意义。Objective To analyze the characteristics of CT signs and pathologic manifestations of small cell lung cancer.Methods CT enhanced performance of 34 cases with pathology-confirmed small cell lung cancer was retrospectively analyzed,focusing on the bronchus,blood vessels,lymphatic tissue,and the pleura invasion.Results In these 34 cases,29 cases with bronchus invasion mainly showed bronchial compression and narrow and,lately,bronchial obstruction occurred;Vascular invasion was found in 24 cases,which was characterized by vascular compression of the violation,with frozen shape.When the mass and the enlarged hilar plus mediastinal lymph nodes were fused,a ＂frozen mediastinum＂ was formed;Lymphoid tissue invasion was found in all the 34 cases,which was characterized by the linear shadow and line spacing radiating from the mass to the surrounding lung field,granular shadow,the mediastinum,hilar and chest-outside lymph node metastasis;17 cases involved in pleural invasion,in which 12 cases presented with pleural effusion（6cases on both sides）,and 5 cases with pleural thickening.Conclusion The analyses of characteristics of CT signs and pathologic manifestations of small cell lung cancer,especially,the manifestations of the bronchus,blood vessels,lymphatic tissue,and the pleura invasion,are useful for the early diagnosis and staging of the small cell lung cancer.
