前列地尔注射液对重度急性胰腺炎并发肝损害的保护作用

摘　　要：背景:重度急性胰腺炎(SAP)是以胰腺自身消化为基础和胰腺自身坏死为主要特征的一种全身性疾病,可导致多器官功能不全综合征,临床上尤以肝损害最为显著。目的:探讨前列地尔注射液对SAP并发肝损害的保护作用。方法:纳入2012年10月-2014年4月浙江省中医院收治的SAP患者36例,随机分为试验组和对照组,对照组给予SAP常规治疗,试验组在常规治疗的基础上予以前列地尔注射液。所有患者于治疗前和治疗后7 d采集静脉血,检测血清ALT、AST、TBIL、DBIL水平。结果:治疗前试验组血清ALT、AST、TBIL、DBIL水平与对照组相比差异无统计学意义(P>0.05),治疗后两组血清ALT、AST、TBIL、DBIL水平均显著降低(P<0.05),且试验组血清ALT、AST、TBIL水平显著低于对照组(P<0.05),DBIL水平在两组间差异无统计学意义(P>0.05)。结论:前列地尔注射液对SAP患者的肝功能具有保护作用。Severe acute pancreatitis （SAP） is a systemic disease with pancreatic autodigestion and pancreatic necrosis, it can lead to multiple organ dysfunction syndrome, especially hepatic injury. Aims： To investigate the protective effect of alprostadil injection on hepatic injury in patients with SAP. Methods： A total of 36 SAP patients from October 2012 to April 2014 at Zhejiang Provincial Hospital of TCM were enrolled and randomly assigned into treatment group and control group. Patients in control group received conventional treatment for SAP, patients in treatment group received alprostadil injection in addition to conventional treatment. Blood samples were collected before and 7 days after the treatment, and serum levels of ALT, AST, TBIL and DBIL were determined. Results： Before treatment, no significant differences in serum levels of ALT, AST, TBIL and DBIL were found between treatment group and control group （P 〉 0.05）. After treatment, serum levels of ALT, AST, TBIL and DBIL were significantly decreased in both two groups （P 〈 0.05）, however, serum levels of ALT, AST and TBIL in treatment group were significantly lower than those in control group （P 〈 0.05 ）, but no significant difference in level of DBIL was found between the two groups （ P 〉 0.05 ）. Conclusions： Alprostadil injection has protective effect on liver function in patients with SAP.
