硅油填充在病理性高度近视眼黄斑裂孔性视网膜脱离术后的1年观察

摘　　要：目的通过对病理性高度近视眼黄斑裂孔性视网膜脱离(MHRD)的患者进行玻璃体切除术后填充硅油,主要观察和探讨此类疾病玻璃体手术填充硅油后视网膜的首次复位率和手术并发症,为此类手术后玻璃体腔填充材料提供临床指导依据。方法观察玻璃体切割联合眼内填充硅油的病理性高度近视MHRD患者,对手术后1周及1、3、6、12个月,视网膜复位率和眼压进行研究,观察患者视网膜复位及眼压的情况,得出个体化的填充方式。结果初次手术后,视网膜复位率为88.0%,半年后硅油取出,视网膜总复位率为75.0%;高眼压的发生在1周内最多,多数药物控制良好。结论病理性高度近视MHRD患者玻璃体手术联合硅油填充视网膜复位率较高,高眼压的发生较常见,但药物控制良好,无其它严重并发症发生。Objective To observe the rate of retinal reattachment and incidence of high intraocular pressure （IOP） after vitrectomy combined with silicone oil tamponade for macular hole retinal detachment （MHRD） in pathologic high myopic eyes. Methods The patients who underwent vitrectomy with silicone oil tamponade were followed at 1 week, 1 month, 3 month, 6 month, and 12 month after surgery. The rate of retinal reattaehment and incidence of lfigh IOP were analyzed. Results After initial surgery, retinal reattachment was achieved in 88.0% of 34 patients. The rate dropped to 75.0% after silicone oil was removed 6 months later. High intraocular pressure was most common within one week, hut can be well controlled with tropical drugs. Conclusion The rate of retinal reattachment after vitreous surgery combined with silicone oil taraponade was relatively high in patients with MHRD and high myopia. Increased intraocular pressure was com- mon after surgery, hut can be well controlled by drugs. There is no other serious complication.
