摘　　要：目的评价西藏高原地区眼内异物伤后万古霉素早期玻璃体内注射预防感染性眼内炎的效果。方法眼内异物伤105例（105眼），依时间先后分为两组。研究组：55例（55眼），进行急诊清创缝合术，眼内异物电磁铁吸出术，玻璃体内注药术（术中注射万古霉素1mg），部分病例转院行异物摘出术。术后全身及局部抗感染治疗。对照组：50例（50眼），系以前住院的患者，那时未行玻璃体内注药术，其余治疗措施与研究组相同，作为对照组。比较两组感染性眼内炎的发生率。结果研究组55例中，术后3例（5．45％）出现感染性眼内炎。对照组50例中，术后9例（18．00％）出现感染性眼内炎。两组病例感染性眼内炎的发生率差异有统计学意义（x^2=4．072，P〈0．05）。结论眼内异物伤后早期玻璃体内注射万古霉素，可减少后期感染性眼内炎的发生率。Objective To evaluate the efficacy of early intravitreal Vancomycin injection for prevention of infectious endophthalmitis after intraoeular foreign body injury. Methods One hundred and five eyes of 105 cases with intraocular foreign body injury were divided into two groups. Fihy-five cases were recruited in research group, before suturing the eyeball wound, the foreign body was attracted with magnet firstly, then intravitreal Vancomycin（ 1 mg） injection was performed and the broad spectrum antibiotics was used after the operation. Some cases failed to attract the foreign body were transferred to another hospital for vitrectomy. In control group, a similar treatment except for intravitreal Vancomycin injection was carried out in 50 cases. The incidences of infectious endophthalmitis between the two groups were compared. Results After operation, infectious endophthahnitis occurred in 3 cases（5.45% ） in research group （55 cases）, in- fectious endophthahnitis occurred in 9 cases（ 18.00% ） in control group （50 cases）. The difference in inci- dence of infectious endophthahnitis between the two groups was statistically significant （X^2 = 4. 072, P 〈 0.05 ）. Conclusion Early intravitreal Vancomycin injection can reduce the incidence of incidence of infectious endophthalmitis after intraocular foreign body injury.
