高度近视伴白内障的“单眼视”法手术的研究

摘　　要：目的探讨超声乳化人工晶状体植入术应用“单眼视”（monovision）法治疗高度近视伴白内障的临床疗效。方法2011年5月至2014年1月36例（72眼）高度近视伴白内障行超声乳化人工晶状体植入术。根据患者对视觉的不同要求分为两组：单眼视组18例（36眼），主导眼矫正至术后看远，非主导眼矫正至术后看近；对照组18例（36眼），双眼均矫正至术后看远。观察术后双眼屈光状态、裸眼远近视力、立体视及眼底情况并调查戴镜情况，对数据进行统计学分析。结果两组术后屈光度与术前预留屈光度在预计的范围内，两组术后3个月双眼同时视裸眼远视力0．5～0．8，均不戴镜，单眼视组双眼同时视裸眼近视力优于对照组，差异有统计学意义（P〈0．05），术后近距离用眼时单眼视组基本不戴镜，对照组大部分戴镜，差异有统计学意义（P〈0．05）。结论超声乳化人工晶状体植入术应用“单视眼法”手术治疗高度近视伴白内障是一种有效的方法，且具有术后看远看近完全脱镜的优点。Objective To observe the efficacy of monovision for correcting cataract with high myopia by phacoemulsifieation with intraocular lens implantation. Methods Phacoemulsifieation with intraocular lens implantation was applied on 36 patients （72 eyes ） who suffered from cataract with high myopia from May 2011 to January 2014 in our department. According to monovision request, we divide the patients into two groups, monovision group （Group M） and reference group （Group R）, 18 patients （36 eyes） in each group. For Group M, we correct the dominant eyes for far vision while the non-dominant eyes for near vision after surgery. For Group R, we correct both eyes for far vision. The refractive status, far and near vision of uncorrected eyes, stereoscopic vision, fundus and the situation of wearing glasses were observed. The data were analyzed statistically. Results In both groups, the postoperative refractive value was within planed range. The far vision of uncorrected eye was 0.5-0.8 three months after the surgery. The uncorrected near vision in Group M was better that that of Group R, with statistical difference （ P 〈 0.05 ）. Patients in Group M can basically get rid of glasses for near vision after surgery, while patients in Group R still have to wear glasses, with statistical difference （P 〈 0. 05 ）. Conclusion Monovison after phacoemulsification with intraoeular lens implantation is an efficient, and safe treatment for cataract with high myopia. It take the advantage to get rid of glasses after surgery.
