超声引导下粗针穿刺活检在腮腺区肿块诊断中的意义

摘　　要：目的探讨彩色多普勒超声引导下粗针穿刺活检术(USCB)在腮腺区肿块诊断中的应用价值。方法回顾性地研究2005年8月至2014年3月在南方医科大学附属南方医院行彩超引导下粗针穿刺活检的53例腮腺区肿块患者的病理结果;对其中43例需行手术治疗的患者,将术前穿刺活检病理结果与术后石蜡病理结果对比分析其诊断准确率、特异性和敏感性,所得数据采用SPSS 17.0软件包进行分析。结果 53例患者全部穿刺成功,穿刺成功率为100%,无明显并发症发生;10例腮腺区非肿瘤患者采用保守治疗,效果满意;以术后病理结果为标准,USCB对腮腺区肿瘤的诊断特异性和敏感性均为100%;良性肿瘤的准确性为96.8%(30/31),恶性肿瘤的准确性为91.7%(11/12),总的准确性为95.3%(41/43),USCB与术后病理的一致性较高((Kappa=0.834)。结论对于腮腺区肿块的诊断,USCB具有简便、安全、准确的优点,可以避免不必要的手术创伤,可作为腮腺区肿块的常规检查方法。Objective To evaluate the value of color Doppler ultrasound-guided core biopsy（USCB）in the diagnosis of palpable masses in parotid region.Methods Pathologic data of 53 patients with masses in parotid region who underwent USCB in Nanfang Hospital Affiliated to Southern Medical University between August 2005 and March 2014 were retrospectively analyzed.The pathologic results achieved by preoperative USCB were compared with those achieved by postoperative paraffin-embedded tissue pathology in 43 patients who underwent surgical treatment to analyze the diagnostic accuracy,specificity and sensitivity of USCB.Data were analyzed using SPSS 17.0software.Results The USCB was successfully performed and no obvious complications occurred in all the 53 patients.Ten non-tumor patients received conservative treatment and achieved satisfactory results.Using postoperative pathologic results as the standard,both the specificity and sensitivity of USCB were 100%for the diagnosis of masses in parotid region.The diagnostic accuracy was 96.8%（30/31）for benign tumor and 91.7%（11/12）for malignant tumor.The total accuracy was 95.3%（41/43）.The results of USCB were highly consistent with postoperative pathology（Kappa=0.834）.Conclusion USCB is a simple,safe and accurate method for the diagnosis of masses in parotid region and can avoid unnecessary surgical trauma.Therefore,USCB may be used as a routine examination procedure in the diagnosis of masses in parotid region.
