NP方案联合扶正祛痰方治疗非小细胞肺癌晚期的临床研究

摘　　要：目的:研究NP方案(长春瑞滨联合顺铂)联合扶正祛痰方治疗晚期非小细胞肺癌的有效性及安全性。方法:选取2011年1月—2014年6月于医院住院接受治疗的晚期非小细胞肺癌患者为研究对象,共94例,将患者按随机数字法分至观察组和对照组,各47例,观察组予以NP方案联合扶正祛痰方治疗,对照组仅予以NP方案化疗,比较两组患者治疗的临床疗效、中医临床证候改善情况、生活质量改善率及药物不良反应发生情况。结果:观察组患者治疗有效率、稳定率、中医临床证候改善有效率分别为29.79%、76.60%、85.11%,明显高于对照组的23.40%、51.06%、55.32%,组间存在显著性差异(P<0.05)。两组患者治疗后心理状况、功能状况、肺癌附加关注及生活质量改善总评分较治疗前增加,组间存在显著性差异(P<0.05),观察组治疗后情感评分较治疗前亦明显升高(P<0.05)。观察组患者治疗后心理状况、情感、功能状况、肺癌附加关注及生活质量改善总评分均明显高于对照组,组间存在显著性差异(P<0.05)。观察组患者恶心呕吐、口腔黏膜炎、肝损害、白细胞减少不良反应发生率均明显低于对照组,组间存在显著性差异(P<0.05),但两组患者肾损害及血小板降低发生率无明显差异(P>0.05)。结论:NP方案联合扶正祛痰方用于治疗晚期非小细胞肺癌患者时,可明显提高临床疗效,改善患者临床证候,提高日常生活质量,降低药物不良反应发生率。Objective： To study the effect and safety of NP scheme combined with Fuzheng Qutan Prescription in the treatment of advanced non small cell lung cancer. Methods： From 2011 January- June 2014 in our hospital,a total of 94 cases of advanced non small cell lung cancer patients were randomly divided to observation group and control group,47 cases in each. The observation was treated by NP regimen combined with Fuzheng Qutan Prescription and control group was given NP chemotherapy. The clinical efficacy,clinical syndromes improvement,the quality of life and the occurrence rate of adverse reactions were compared between two groups. Results： The observation group＇s treatment efficiency,stable rate,clinical syndrome improving were respectively 29. 79%,76. 60% and 85. 11%,significantly higher than 23. 40%,51. 06%,55. 32% in control group and there was significant difference between two groups（ P〈 0. 05）. Two groups＇ life quality of lung cancer psychological status,functional status and the total score were increased than those before treatment and there were significant differences between the two groups（ P〈 0. 05）. The observation group＇s emotional scores after treatment increased significantly（ P〈 0. 05）. The observation group＇s additional attention and life quality,emotions,psychological conditions and the total score were significantly higher than those in the control group and there was significant difference between the two groups（ P〈 0. 05）. The observation group＇s nausea and vomiting,mucositis,liver damage,white blood cells and the occurrence rate of adverse reaction were lower than those of the control group and there was significant difference between the two groups（ P〈 0. 05） but the kidney damage and decreased platelet counts in two groups showed no significant difference（ P〈 0. 05）. Conclusion： NP regimen combined with Fuzheng Qutan Prescription for the treatment of advanced non small cell lung cancer patients can significantly improve the clinical efficac
