DERMOSCOPY-Ⅱ皮肤镜图像诊断工作站的临床应用价值研究

摘　　要：目的探讨"DERMOSCOPY-Ⅱ皮肤镜图像诊断工作站"(简称"工作站")对某些色素性皮肤肿瘤和白癜风的临床诊断价值。方法收集色素性皮肤肿瘤和白癜风患者共76例。首先,临床肉眼判断,后经"工作站"检查分析,最终组织病理验证,白癜风皮损经伍德氏灯检查验证,进行统计学处理和分析。结果经"工作站"诊断,符合73例,不符合3例,分别是表皮囊肿、色素性日光角化病和色素性鲍温氏病;其平均诊断符合率达96.05%,比临床肉眼判断提高了29.94%;色素性皮肤肿瘤良、恶性灵敏度93.75%、特异度95.45%。结论 "工作站"对某些色素性皮肤肿瘤的良、恶性诊断和白癜风判断具有较好的辅助性和临床应用价值。Objective To evaluate the value of DERMOSCOPY-Ⅱ imaging diagnostic workstation （work- station for short） for the diagnosis of certain pigmented skin tumors and vitiligo. Methods We collected seventy-six cases of pigmented skin tumors and vitiligo, diagnozed firstly by clinical judgment, and then analyzed the lesions by the workstation, respectively, and finally verified the tumors and vitiligo by histo-pathology and Wood＇slamp examination. The data were statistically analyzed. Results Among the 76 ca- ses, the diagnosis of 73 cases was accurate and the inconsistent ones included one case of epidermal cyst, pigmented actinic keratosis and pigmented Boweng disease, each. The average diagnostic accordance rate reached 96.05% , i. e. 29.94% higher than that of clinical judgment. The diagnostic sensitivity and spe- cificity of benign and malignant pigmented skin tumors was 93.75% and 95.45%, respectively Conclusions The workstation possesses preferable auxiliary effect and clinical application value for the diagnosis of benign or malignant pigmented skin tumors and vitiligo
