摘 要:目的:初步探讨人根尖周炎中亲环素A(Cyp A)表达及与临床表现的关系。方法:收集根尖手术切除并经病理诊断证实的根尖周炎病损组织83例作为实验组;收集新鲜拔除的健康正畸牙牙髓6例作为正常对照组。用免疫组织化学染色法分析比较各组织中Cyp A及RANKL的表达情况,取蛋白平均吸光度值(A值)进行统计学分析,并比较其表达量与临床症状的相关性。结果:根尖周炎病变组织中Cyp A主要表达于炎细胞浸润区和上皮组织,主要阳性细胞为淋巴样细胞、上皮细胞、血管内皮细胞,正常对照组中少见Cyp A的表达;Cyp A在根尖肉芽肿组织中的表达量明显高于根尖囊肿组(P<0.05),RANKL在根尖囊肿组织中的表达量明显高于根尖肉芽肿组(P<0.05);有明显临床症状的根尖周炎组织中Cyp A和RANKL的A值均明显高于无症状组(P<0.05);Cyp A和RANKL的表达水平与根尖周炎病变大小均呈正相关(r=0.462,P<0.05;r=0.417,P<0.05)。结论:Cyp A存在于人根尖周炎病损组织中,可能参与人根尖周炎疾病的发病进程。AIM: To examine the expression of cyclophilin A( Cyp A) in human periapical lesions and to investigate its relationship with the clinical symptoms of periapical periodontitis. METHODS: Samples of human apical periodontitis lesions of 83 cases were subjected to immunohistochemical examination for Cyp A and RANKL expression. Six 6 healthy dental pulp samples obtained from orthodontic patients served as the controls. The integrated average optical density value( A) of Cyp A and RANKL was measured by immunohistochemical analysis for all the samples.Differences in Cyp A and RANKL expression between different clinical presentations were subsequently analyzed by t-test. RESULTS: Cyp A postitive cells and RANKL positive cells were both detected in all periapical lesion tissues,and the expression of Cyp A was significantly higher in periapical lesions than in normal tissues( P〈0. 05). Epithelial cells,lymphoid cells and endothelial-like cells showed Cyp A positive in the lesions. The expression of RANKL in cyst group were higher than in granuloma group( Z = 4. 48,P〈0. 05),while the expression of Cyp A in granuloma group were higher than in cyst group( Z = 4. 48,P〈0. 05). The average( A) value of Cyp A positive sites in periapical lesions with clinical symptoms was higher than that without clinical symptoms( 77. 24 versus 39. 18,P〈0. 05). Positive correlation was observed between Cyp A expression and lesion size( r = 0. 462,P〈0. 05). Moreover,positive correlation between RANKL expression and lesion size was observed( r = 0. 417,P〈0. 05). CONCLUSION: Cyp A might be involved in the inflammatory response and bone resorption,and associated with periapical lesion pathogenesis.
