摘 要:目的用有限元方法分析比较利用组牙支抗和微种植体支抗远移下颌第二磨牙时牙齿的位移及应力分布情况。方法采用螺旋CT扫描,结合Mimics、Ansys软件建立3组有限元模型,组牙支抗组和微种植体支抗组为实验组,前牙无整体连续结扎无微种植体组为对照组,并对各组模型进行相应力值加载和计算。结果组牙支抗组支抗牙初始位移值及应力值较对照组减小,但相差不大。微种植体支抗组中切牙至第二前磨牙初始位移值及应力值均明显小于对照组。下颌第一磨牙牙冠初始位移增大,但牙根初始位移明显减小,下颌第二磨牙初始位移大于对照组。3组模型中下颌第二磨牙远中倾斜伴舌侧扭转运动。结论在推下颌第二磨牙远移过程中,将前牙整体结扎并未能明显增强支抗,微种植体支抗能够提供较强的支抗控制。Objective To investigate the displacement and stress distribution of tooth during the mandibular second molar distalization compared with micro-implant anchorage and group dental anchorage by using the finite element method. Methods The spiral CT scanning and Mimics,Ansys software were used to establish the three finite element models. Group dental anchorage and micro- implant anchorage were the experimental group,and the anterior teeth or micro implant served as control. For each model,the corresponding loading and calculation were conducted. Results Initial displacement value and stress of anchorage tooth reduced in group dental anchorage group than in the control group,but the difference was not significant. Initial displacement value and stress of the incisor to the second premolar were significantly lower in micro-implant group than in the control group. The initial displacement value of the crown of the mandibular first molar increased,but the initial displacement value of the root reduced,and the initial displacement value of mandibular second molar increased. Mandibular second molar tilted distally and twisted lingually in each group. Conclusion The front teeth horoscopes ligation did not significantly enhance anchorage. Micro-implant anchorage can provide strong anchorage control in pushing mandibular second molar distally.
