晚期非小细胞肺癌患者化疗前后血清HIF-1α和VEGF的变化及临床意义

摘　　要：目的:探讨晚期非小细胞肺癌(non-small cell lung cancer,NSCLC)患者化疗前后外周血清缺氧诱导因子-1α(hypoxia inducible factor-1,HIF-1α)、血管内皮生长因子(vascular endothelial growth factor,VEGF)与化疗疗效之间的关系。方法:选择本院收治的22例NSCLC患者,采用酶联免疫吸附试验法(enzyme linked immunossorbent assay,ELISA)检测患者化疗前后血清中HIF-1α和VEGF的含量。结果:本组患者化疗总有效率为27%。化疗后患者血清HIF-1α和VEGF与化疗前比均有升高趋势,化疗无效组血清VEGF由(562.03±279.10)pg/ml升高为(740.26±364.34)pg/ml,差异显著(P<0.05)。化疗前血清HIF-1α和VEGF水平之间,及与血清癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、细胞角蛋白(CYFRA)21-1之间无显著相关性。化疗前血清HIF-1α和VEGF含量在年龄、性别、病理类型、分期、有无胸水、疗效上无明显统计学意义。结论:检测晚期NSCLC患者外周血VEGF,可能有利于协助评估化疗疗效。Objective：To investigate the changes of serum hypoxia inducible factor-1α（HIF-1α） and vascular endothelial growth factor（ VEGF） before and after chemotherapy in patients with non-small cell lung cancer（NSCLC）.Methods：The serum levels of HIF-1α and VEGF in 22 cases with advanced NSCLC were measured by enzyme linked immunossorbent assay（ ELISA） before and after chemotherapy.Results：The total clinical treatment efficacy of patients was 27%.The serum levels of HIF-1α and VEGF were elevated after chemotherapy,but only the expression of serum VEGF was significantly higher after chemotherapy（ 740.26 ± 364.34） pg / ml than that before chemotherapy（562.03 ± 279.10） pg / ml in the ineffective cases（ P〈0.05）.No significant correlation was found between serum HIF-1α and VEGF,and between serum HIF-1α or VEGF and tumor markers before chemotherapy.There were no differences in the pretreatment levels of HIF-1α and VEGF among the following clinical parameters such as age,gender,histology subtype,stage,with or without metastatic pleural effusion,and treatment response.Conclusion：The measurement of serum VEGF before and after chemotherapy might be helpful in the evaluation of treatment outcomes.
