麻黄碱伍用丙泊酚、芬太尼在胃镜检查术中安全性和麻醉效果的Meta分析

摘　　要：目的:系统评价麻黄碱复合丙泊酚、芬太尼在胃镜检查术中的安全性和临床麻醉效果。方法:计算机检索Pub Med、EMbase、CBM、CNKI、VIP和Wan Fang Data关于麻黄碱复合丙泊酚、芬太尼用于无痛胃镜检查的随机对照实验(RCT)。由2位作者按照纳入与排除标准独立筛选文献,提取资料和评价质量,然后用Rev Man 5.2进行Meta分析。结果:最终纳入9个RCT。Meta分析结果显示:1在丙泊酚、芬太尼基础上加用麻黄碱后,患者心率(HR)、呼吸频率(RR)、血氧饱和度(Sp O2)、收缩压(SBP)均更稳定,两组间差异有统计学意义。2术后恢复时间:两组间在呼之睁眼和能正确对答的时间上无统计学差异;加用麻黄碱后自行离开时间比对照组短,两组间差异有统计学意义。3视觉模拟评分(VAS):两组术后视觉模拟评分差异无统计学差异。结论:基于现有证据,丙泊酚联用芬太尼基础上加用麻黄碱用于胃镜检查时安全性更好。Objective： To systematically review the safety and clinical effectiveness of ephedrine with fentanil- propofol versus fentanil- propofol for gastroscopy. Methods： Databases including Pub Med,EMbase,CBM,CNKI,VIP and Wan Fang Data were electronically searched for the randomized controlled trials of ephedrine with fentanil- propofol versus fentanil- propofol for gastroscopy. Two reviewers independently screened literatures according to the inclusion and exclusion criteria,extracted data,and assessed the quality of included studies. Then,meta- analysis was performed by using Rev Man 5. 2 software. Results： 9 RCTs were finally included. The results of meta- analysis showed that： a） Compared with the fentanil- propofol group,ephedrine with fentanil- propofol could maintain significantly stable heart rate,breathing rate,oxygen saturation of blood and systolic blood pressure. b） For recovery time,time for emergence and clear- headed had no significant difference between two groups,and the leaving hospital time was shorter in the ephedrine with fentanil- propofol group than that in the fentanil- propofol group. c） VAS,there was no significant difference found between two groups. Conclusion： The results of meta- analysis indicate that ephedrine with fentanil- propofol for gastroscopy has better safety.
