急性心肌梗死后MMP-9与心室重构的相关性

摘　　要：目的观察急性心肌梗死（AMI）后基质金属蛋白酶-9（MMP-9）表达变化,探讨其是否与AMI后心室重构相关。方法通过免疫组化检测大鼠AMI后心肌组织中的MMP-9,明确其表达及定位;筛选2012年10月~2013年8月在川北医学院附属医院救治,符合要求的左室前壁AMI患者40例,健康对照组20例,用夹心酶联免疫吸附法（ELISA）检测受试者静脉血清中MMP-9水平;二维超声检测AMI后7天左心室舒张末内径（LVEDD）。结果 MMP-9大量表达于AMI大鼠心肌细胞胞浆;AMI组患者外周血清MMP-9表达显著高于对照组（P〈0.01）;AMI患者在发病第3、5、7和10天的血清MMP-9水平存在显著差异（均P〈0.01）,且呈逐渐升高趋势;AMI组患者病程第7天时MMP-9水平与此时检测的LVEDD呈线性相关（r=0.869,P〈0.01）。结论 MMP-9可能参与AMI后心室重构。Objective To investigate the changes of plasma MMP-9 in acute myocardial infarction（AMI）patients and access the value of MMP-9 in post-AMI LV remodeling.Methods AMI model in vivo rats were established.Myocardial MMP-9 proteins were detected by immunohistochemical method.40 patients with AMI were included（AMI group）and 20 healthy persons served as the control group.Serum MMP-9 were examined in these subjects using enzymelinked immunosorbent assay（ELISA）.Left ventricular end diastolic diameter（LVEDD）was detected using 2DE.Results The MMP-9 of myocardic cytoplasm in AMI group was significantly higher than that in control group（P〈0.01）.MMP-9level was remarkable elevated in different period of AMI.It was showed linear dependence between MMP-9 level and LVEDD in the AMI patients on the seventh day（r=0.869,P〈0.01）.Conclusion MMP-9 might be involved in LV remodeling post-AMI.
