远端胃癌腹腔镜D_2根治术近期疗效分析

摘　　要：目的:探讨远端胃癌腹腔镜D2根治术的近期疗效及手术安全性。方法:选择2012年6月至2013年9月43例行胃癌D2根治术的远端胃癌患者,根据手术方式分为腹腔镜组(n=18)与开腹组(n=25)。对比两组患者术中及术后恢复情况、近期疗效及并发症等。结果:两组患者手术时间、淋巴结清除数量差异无统计学意义(P>0.05);腹腔镜组术中出血量、术后肛门排气时间及并发症发生率明显优于开腹组,差异有统计学意义(P<0.05);结论:与开放手术相比,腹腔镜远端胃癌D2根治术具有患者创伤小、康复快、出血少等优点,手术疗效及安全性可靠。Objective： To investigate the short-term therapy efficacy and safety of laparoscopic D2 radical operation for distal gastric cancer. Methods： From Jun. 2012 to Sep. 2013,43 patients receiving D2 radical operation for distal gastric cancer were divided into laparoscopic group（ n = 18） and open surgery group（ n = 25）. Intraoperative and postoperative recovery,short-term therapy effects and complications were compared between the two groups. Results： There was no significant difference in operation time and the number of lymph node dissection between the two groups（ P〉0. 05）. Intraoperative bleeding volume,postoperative anal exhaust time and complication incidence in laparoscopic group were significantly better than those in open surgery group（ P〈0. 05）. Conclusions： Compared with open surgery,laparoscopic D2 radical operation for distal gastric cancer has advantages of minimal invasion,rapid recovery,little blood loss,reliable efficacy and safety.
