三药联用改良湿敷法在治疗新生儿强刺激性药物外渗损伤的应用

摘　　要：目的探讨三药（50％葡萄糖、维生素B12、地塞米松）联用改良湿敷法治疗强刺激性药物造成外渗性皮肤损伤的临床效果。方法选择2013年5月∽12月发生强刺激性药物外渗损伤患儿32例作为对照组，常规应用50％硫酸镁患处湿敷。2014年1月∽8月发生强刺激性药物外渗损伤患儿35例作为治疗组，采用50％葡萄糖联合维生素B12持续湿敷，在湿敷间隙予地塞米松原液外涂待干的方法，循环进行。治疗结束后分别观察治疗效果。结果治疗组临床总有效率为100％，显效率为80％；对照组总有效率为78．12％，显效率为43．75％，两组比较有显著性差异（P＜0．05），两组肿胀消失时间比较有显著性差异（P＜0．05）。结论使用三药联用改良湿敷法，能有效治疗强刺激性药物渗漏对患儿皮肤及皮下组织造成的损害，可在临床推广。Objective To explore the three drugs （50% glucose ,vitamin B12 ,dexamethasone） combined with impro‐ving clinical effect of wet dressing therapy and strong irritant drugs caused by extravasation of skin injury .Method Choose 32 cases of drug extravasation injury in neonatal strong stimulation occurred from May to December ,2013 as the control group ,50% routine application of the affected area Magnesium sulfate wet compress .January to August will be a strong ir‐ritant drugs extravasation injury in 35 patients as the treatment group ,the treatment group with 50% glucose combined with vitamin B12 sustained wet packing ,method in wet compress the gap to the dexamethasone solution coated with the dry ,cy‐cle .To observe the curative effect after the treatment respectively .Result The clinical efficiency of the treatment group was 100% ,markedly effective rate was 80% ;the control group the total effective rate was 78 .12% ,markedly effective rate was 43 .75% , the two groups after treatment efficiency and significant efficiency were statistically significant difference （P〈0 .05）and 17 cases of treatment group 24 hours swelling disappeared persons ,7 cases in the control group .The differ‐ence was statistically significant （P〈0 .05） .Conclusion The use of three drugs improved wet dressing therapy can be effec‐tive in the treatment of strong irritant drugs leakage caused by the skin and subcutaneous tissue of neonatal damage ,worth in clinical promotion.
