奥沙利铂分别联合卡培他滨及5-氟尿嘧啶/亚叶酸钙治疗晚期胃癌的疗效观察

摘　　要：目的比较奥沙利铂联合卡培他滨（XELOX）与奥沙利铂联合5-氟尿嘧啶/亚叶酸钙（FOLFOX4）方案治疗晚期胃癌的疗效及毒副反应。方法 2006年5月至2013年9月经病理组织学确诊为转移或复发晚期胃癌患者75例,采用XELOX方案治疗37例（XELOX组）,采用FOLFOX4方案治疗38例（FOLFOX4组）。两组患者均治疗2周期以上。观察两组疗效及毒性反应。结果两组有效率、疾病控制率、近期客观疗效差异无统计学意义。XELOX组恶心呕吐、血液学毒性、口腔炎、脱发、高胆红素血症的发生率低于FOLFOX4组（P〈0.05）,手足综合征发生率明显高于FOLFOX4组（P〈0.05）,但程度主要为Ⅰ-Ⅱ度。两组周围神经毒性和血小板减少发生率比较,差异均无统计学意义（P〉0.05）。结论 XELOX与FOLFOX4方案治疗晚期胃癌疗效相近,但XELOX方案用药更方便,安全性更好,值得临床一线使用。Objective To compare the efficacy and toxicity of a regimen of capecitabine combined with oxaliplatin（ XELOX）with a regimen of oxaliplatin combined with 5-fluorouracil / leucovorin（ 5-FU / LV）（ FOLFOX4） in the treatment of advanced gastric cancer. Methods Seventy-five patients with metastastic or recurrent gastric cancer were enrolled in this study. Patients were randomly divided into XELOX（ n = 37） or FOLFOX4（ n = 38） groups,which were treated with XELOX or FOLFOX4 regimens,respectively. All patients received two or more cycles of the chemotherapy. The efficacy and toxicity were compared between the two groups. Results There was no statistical difference in the efficiency,disease control rate and short- term objective efficiency between the two groups.However,the incidence of nausea,vomiting,hematological toxicity,alopecia and hyperbilirubinemia in the XELOX group was significantly higher than that in the FOLFOX4 group（ P〈0. 05）. The incidence of hand-foot syndrome was also higher in the XELOX group than that in the FOLFOX4 group（ P〈0. 05）,but the severity was only grade I or II. The incidence of peripheral neurotoxicity and thrombocytopenia was similar between the two groups（ P0. 05）. Conclusion The efficiency rate is not markedly different between the two regimens. However,XELOX regimen is safer and its use is more convenient than FOLFOX4 regimen. Therefore,XELOX regimen is worth recommending as a first-line therapy for the treatment of advanced gastric cancer patients.
