认知行为疗法联合药物治疗惊恐障碍疗效Meta分析

摘　　要：目的:比较认知行为疗法(CBT)联合药物治疗与单纯药物治疗对惊恐障碍的疗效。方法:对CBT联合药物治疗与单纯药物治疗惊恐障碍的对照研究进行系统评价,使用Revman 5.2进行Meta分析。结果:共纳入6项符合标准的随机对照研究,合计样本量329人。6组数据比较了CBT联合药物治疗与单纯药物治疗惊恐障碍的有效率和显效率,结果显示,联合治疗组的有效率(OR=3.28,Z=2.75,P<0.05)与显效率(OR=2.85,Z=3.90,P<0.05)均优于单纯药物组;5组数据比较了CBT联合药物治疗与单纯药物治疗惊恐障碍的治愈率,结果显示,联合治疗组的治愈率优于单纯药物组(OR=2.08,Z=2.82,P<0.05)。结论:鉴于目前结果推测对于惊恐障碍的改善,认知行为疗法联合药物治疗优于单纯药物治疗,但仍需进一步研究证实。Objective：To evaluate the efficacy of cognitive behavioral therapy （CBT）combined with pharmacotherapy in the treatment of panic disorder （PD）. Methods：The randomized controlled trials （RCTs） comparing the efficacy of cognitive behavioral therapy（CBT） combined with pharmacotherapy in the treatment of panic disorder（ PD）were systematically evaluated. The statistical analysis soft ware Rev- Man 5.2 was used to Meta - analysis. Results： Six studies were involved in the Meta - analysis, including 329 patients with PD. There were 6 sets of data which compared the total efficiency and significant effi- ciency of combined therapy and pharmacotherapy alone, combined therapy were superior to pharmacother- apy alone in both total efficiency（ OR = 3.28, Z = 2.75 ,P 〈 0.05 ）and significant efficiency（ OR = 2.85, Z = 3.90, P 〈 0.05 ）. There were 5 sets of data which compared the cure rate of combined therapy and pharmacotherapy alone, and combined therapy was superior to pharmacotherapy alone in cure rate （ OR = 2.08, Z = 2. 82, P 〈 0.05 ）. Conclusion： It suggests that CBT combined with pharmacotherapy may be su- perior to pharmacotherapy alone in improving panic symptom and further study is necessary.
