改良冷刀锥切与LEEP治疗宫颈高级别上皮内瘤样病变的疗效比较

摘　　要：目的比较改良冷刀锥切（CKC）和LEEP治疗宫颈高级别上皮内瘤样病变（CINⅡ~Ⅲ）的临床疗效。方法对155例CINⅡ~Ⅲ患者行改良CKC或LEEP术,比较两种方法手术时间、术中出血量、术后创面出血发生率、术前与术后病理符合率、病变切缘阳性率、宫颈狭窄或粘连发生率、复发率等。结果 LEEP组不需住院治疗,其平均手术时间明显短于改良CKC组（P〈0.01）,术中出血量明显少于改良CKC组（P〈0.01）,但术后出血发生率明显高于改良CKC组（P〈0.05）。两组术后病变残留、与术前病理符合、宫颈狭窄或粘连及复发率比较差异均无统计学意义（P〉0.05）。结论改良CKC与LEEP临床疗效相似,两种方法各有优劣,临床工作中需依据患者的具体病情选择合适的治疗方法。Objective To compare clinical values of improved cold knife conization（ CKC） and LEEP conization in the treatment of high-grade cervical intraepithelial neoplasia（ CINⅡ ~ Ⅲ）. Methods Improved CKC or LEEP conization were carried out to 155 cases of CINⅡ ~ Ⅲ. And operation time,amount of intraoperative blood loss,postoperative bleeding,consistency between preoperative and postoperative pathology,incidence rate and recurrent rate of cervical stenosis or adhesions were compared between the two groups. Results The LEEP group were not required hospitalization,they had shorter operative time and less amount of intraoperative blood loss than the improved CKC group（ P 〈0. 01）,but the rate of postoperative bleeding was higher than that of the improved CKC group,there were statistically significant differences between the two groups（ P 〈0. 05）. The postoperative residual disease,consistency between preoperative and postoperative pathology,cervical stenosis or adhesions and recurrence rate of the two groups showed no statistically significant differences（ P 〉0. 05）. Conclusion There are similar clinical values between improved CKC and LEEP conization,and each method has its own advantages and disadvantages in curing CINⅡ ~ Ⅲ. Thus,they should be chosen in accordance with specific conditions of the patients in clinical practices.
