摘 要:目的分析血清肿瘤相关物质(BXTM)在肿瘤早期诊断及疗效监测中的应用价值。方法收集中南大学湘雅医院2013年4月-2014年2月实验组800例癌症患者标本(肺癌、胃癌、食管癌、宫颈癌各200例)及对照组195例非癌症患者标本(102例健康体检及93例非癌疾患),采用BXTM试剂盒比较BXTM浓度在不同人群中的差异;同时收集各100例四种癌症患者标本,动态监测癌症患者治疗前后BXTM的浓度变化。结果血清BXTM浓度对恶性肿瘤的总体敏感性为75.38%,特异性为82.56%;实验组各肿瘤阳性率均〉70%,其中肺癌最高达78.0%,宫颈癌最低为73.0%,实验组各肿瘤阳性率明显高于对照组,差异具有统计学意义(P〈0.001);治疗后四种癌症患者BXTM浓度较治疗前平均下降10~15 U/ml,差异具有统计学意义(P〈0.001)。结论血清BXTM浓度检测可为肿瘤早期诊断提供有用的参考价值,具有较高的敏感性和特异性,同时可作为病情及疗效的动态监测指标。【Objective】To analyze the application value of Bo Xin tumor associated material(BXTM) in the early diagnosis of tumor and monitoring of the curative effect.【Methods】Eight hundred specimens of the patients with cancer(200 cases of each of the lung cancer, gastric cancer, esophageal cancer and cervical cancer) in the experiment group and 195 specimens of the patients without cancer(102 cases of healthy check-up and 93 cases of noncancer diseases) in the control group were collected from April 2013 to February 2014 at Xiangya Hospital of Central South University. The difference of BXTM concentration in different groups was compared with the method of BXTM kit. Meanwhile 100 specimens were collected from each of four kinds of cancer patients, and the change of BXTM concentration before and after the treatment was dynamically monitored.【Results】The overall sensitivity of serum BXTM concentration in the malignant tumors was 75.38%, and the specificity was 82.56%. The positive rate of each tumor in the experimental group was higher than 70% with the highest positive rate of 78.0% in the lung cancer and the lowest positive rate of 73.0% in the cervical cancer, the positive rate of each tumor in the experimental group was significantly higher than that in the control group with a statistical difference(P〈0.001). The BXTM concentration of the four kinds of cancer after treatment decreased by an average of 10~15 U/ml than before treatment with a significant difference(P〈0.001).【Conclusions】The detection of BXTM concentration in serum can provide a useful reference value for the early diagnosis of tumors with high sensitivity and specificity. It can also be used as a dynamic monitoring index for patient's condition and curative effect.
