GPC3和AFP联合检测对原发性肝癌的诊断价值

摘　　要：目的观察和分析磷脂酰肌醇蛋白聚糖-3(GPC3)和甲胎蛋白(AFP)联合检测对原发性肝癌(PHC)的诊断价值。方法选取60例PHC患者、70例肝硬化患者和50例其它原因致肝破裂患者作为研究对象,对各类患者的血清GPC3和AFP水平和PHC患者的肝组织标本中的GPC3表达水平进行检测。结果肝破裂患者的血清GPC3水平显著低于肝硬化患者(P<0.05),肝硬化患者的血清GPC3水平显著低于PHC患者(P<0.05);应用血清GPC3和AFP联合检测诊断PHC的敏感度为90%,显著高于单独应用血清GPC3检测或血清AFP检测(P<0.05);肿瘤组织的GPC3表达水平显著高于癌旁组织(P<0.05),癌旁组织的GPC3表达水平显著高于正常组织(P<0.05),具有不同Edmondson病理分级或TMN临床分期的PHC患者的肝组织GPC3表达水平的差异有显著性(P<0.05)。结论PHC患者的血清和肿瘤组织中的GPC3均呈现高表达状态,其在肝组织中的表达水平可能与患者病情的进展具有相关性,应用血清GPC3和AFP水平联合检测可提高诊断PHC的敏感度。Objective To observe and analyze the diagnosis value of the combined glypican-3（GPC3）and alpha-fetoprotein（AFP）detection of primary hepatic carcinoma.Methods 60 patients with PHC,70 patients with liver cirrhosis and 50 patients with liver rupture caused by other reasons were selected as the research objects.The serum GPC3 and AFP levels of all kinds of patients and the GPC3 expression level of the liver tissues of the patients with PHC were detected.Results The serum GPC3 level of the patients with liver rupture was significantly lower than that of the patients with liver cirrhosis（P〈0.05）,and the serum GPC3 level of the patients with liver cirrhosis was significantly lower than that of the patients with PHC（P〈0.05）;the sensitivity of the combined serum GPC3 and AFP detection in the diagnosis of PHC was 90%,which was significantly higher than that of single detection of serum GPC3 or serum AFP（P〈0.05）;the GPC3 expression level in the tumor tissues was significantly higher than that in the paracancerous tissues（P〈0.05）,and the GPC3 expression level in the paracancerous tissues was significantly higher than that in the normal liver tissue（P〈0.05）.There was significant difference of the GPC3 expression level in the liver tissue of the PHC patients with different Edmondson pathological grading or different TMN clinical stage（P〈0.05）.ConclusionThe GPC3 levels in serum and tumor tissue of the PHC patients show high expression,and the expression level in liver tissue may be associated with the progress of the diseases.The combined detection of serum GPC3 and AFP levels can improve the diagnostic sensitivity of PHC.
