血清甲壳质酶蛋白40在胃部肿瘤中的表达

摘　　要：目的 探讨胃癌患者血清甲壳质酶蛋白40（YKL-40）表达水平及临床意义.方法 采用酶联免疫吸附试验法（ELISA）检测30例正常人及60例胃癌患者血清中YKL-40的表达水平,并比较36例胃癌患者手术前后血清YKL-40水平.结果 胃癌患者血清YKL-40水平（250.83±84.39） μg/L明显高于正常对照组（72.06 ±17.08） μg/L,两两之间比较差异有统计学意义（P＜0.01）;血清YKL-40水平与胃癌患者性别、年龄无明显相关,与胃癌患者临床分期、肿瘤细胞分化程度有明显相关;Ⅳ期[（362.27±18.64）μg/L]明显高于Ⅲ期[（296.20±25.49） μg/L]、Ⅲ期明显高于Ⅱ期[（212.77 ±24.17） μg/L]、Ⅱ期明显高于Ⅰ期[（136.25 ±20.83） μg/L]、Ⅰ期明显高于正常对照组、两者比较差异有统计学意义（P＜0.01）,肿瘤细胞分化程度较差（低分化腺癌、黏液腺癌）胃癌患者血清YKL-40水平[（284.88±68.96） μg/L]明显高于肿瘤细胞分化程度较好（高分化腺癌、中分化腺癌）胃癌患者[（222.97±86.54） μg/L,P＜0.01];胃癌患者手术后血清YKL-40水平[（191.06±58.33）μg/L]明显低于手术前[（196.14±58.31） μg/L],两者比较差异有统计学意义（P＜0.01）.结论 YKL-40可能参与胃癌的发生发展过程.Objective To investigate serum chitinase protein 40 （YKL-40） levels in patients with gastric carcinoma and its clinical significance.Methods The level of serum YKL-40 was measured by enzyme linked immunosorbent assay （ELISA） in 30 cases of normal control subject and 60 cases of gastric carcinoma,and compared in 36 cases of gastric carcinoma pre-and post-operation.Results The level of serum YKL-40 was elevated in all subgroups of patients with gastric carcinoma [（250.83 ± 84.39） μg/L] compared to the controls [（72.06 ± 17.08） μg/L,P 〈0.01].Serum YKL-40 level was correlated with TNM stage and histological grade of gastric carcinoma,but not with sex and age.The serum YKL-40 level in stage Ⅳ of gastric carcinoma [（362.27 ± 18.64） μg/L] was higher than that in stage Ⅲ [（296.20±25.49） μg/L,P〈0.01],that in stage Ⅲ higher than that in stage Ⅱ [（212.77 ±24.17） μg/L,P 〈0.01],that in stage Ⅱ higher than that in stage Ⅰ [（136.25 ± 20.83） μg/L,P 〈 0.01],and that in stage Ⅰ higher than that in normal controls （P〈0.01）.The level of serum YKL-40 was higher in the cases of poorly differentiated adenocarcinoma and mucoid adenocarcinoma [（284.88 ± 68.96） μg/L] than that in the cases of well differentiated and moderately differentiated adenocarcinoma [（222.97 ±86.54） μg/L,P 〈0.01].The serum YKL-40 levels were remarkably reduced in patients with gastric carcinoma after operation （P 〈 0.01）.Conclusion YKL-40 may take part in the malignant process of gastric carcinoma,and may be as an important marker of judging the prognosis of patients with gastric carcinoma.
