摘  要:目的 探讨人脐带血衍生间质干细胞(UCB-MSCs)在改善大鼠急性期缺血性脑卒中模型神经功能中的作用及机制.方法 82只中动脉闭塞(MCAO)模型大鼠随机分为UCB-MSCs组(n=41)和对照组(n=41),收集脐血并分离培养人UCB-MSCs,进行巢蛋白(Nestin)染色鉴定.将UCB-MSCs移植到造模48 h的大鼠脑室并标记,于移植后24h、7d和14 d时,采用改良大鼠神经功能缺损评分(mNSS)对神经功能进行盲评,采用免疫荧光法对内源性神经干细胞增殖及血管内皮生长因子(VEGF)表达进行检测.结果 42份脐血成功分离UCB-MSCs;移植后7d和14 d时,UCB-MSCs组大鼠模型神经功能评分分别为(6.76 ±0.42)和(4.27 ±0.49)分,均低于对照组,差异均有统计学意义(P<0.05);移植后14 d时,UCB-MSCs组5-溴-2-脱氧尿嘧啶核苷(BrdU)/双肾上腺素激素(DCX)和BrdU/胶质纤维酸性蛋白(GFAP)双重标记阳性细胞数分别为(74.31 ±3.04)和(65.38 ±2.43)个,均高于对照组,差异均有统计学意义(P<0.05);UCB-MSCs组神经营养因子VEGF表达量为109.53 ±5.91,显著高于对照组,差异有统计学意义(P<0.05).结论 UCB-MSCs移植可有效促进大鼠急性期缺血性脑卒中模型神经功能恢复,可能的机制是促进内源性神经干细胞分化及神经营养因子VEGF释放.Objective To investigate the role of human umbilical cord blood (UCB)-derived mesenchymal stem cells (MSCs) in improving the neurological function of acute ischemic stroke model of rats and mechanisms.Methods The middle cerebral artery occulusion (MCAO) models in 82 rats were randomly divided into UCB-MSCs group (n =41) and control group (n =41).Human UCB-MSCs were isolated and cultured,and identified by nestin staining.The UCB-MSCs were transplanted into the ventricles of rats 48 h after modeling and marked.At 24th h,7th day and 14 day after transplantation,the neurological functions were assessed by using improved neurological function in rats (mNSS).The proliferation of endogenous neural stem cells and expression of vascular endothelial growth factor (VEGF) were detected by using immunofluorescence method.Results UCB-MSCs were successfully separated in 42 parts.At 7th day and 14th day after transplantation,the function scores of rats in UCB-MSCs group were 6.76 ± 0.42 and 4.27 ± 0.49,which were statistically significantly lower than in the control group (P 〈 0.05).At 14th day after transplantation,the number of 5-bromo 2'-deoxyuridine (BrdU)/doublecortin (DCX) and BrdU/glial fibrillary acidic protein (GFAP) double labeled positive cells was 74.31 ± 3.04 and 65.38 ± 2.43 respectively,which were significantly greater than in the control group (P 〈 0.05).The expression of neurotrophic factor VEGF in UCB-MSCs group was 109.53 ± 5.91,significantly higher than in the control group (P 〈 0.05).Conclusion UCB-MSCs transplantation could effectively promote neurological function recovery of acute ischemic stroke model possibly by promoting the proliferation of the endogenous neural stem cells and neurotrophic factor VEGF release.
