乌司他丁预防罗库溴铵注射痛的临床研究

摘　　要：目的探讨静脉预先注射乌司他丁预防罗库溴铵注射痛的临床效果。方法选择2013年1～5月雅安市人民医院收治的拟在全身麻醉下行择期手术的患者90例,采用随机数字表法分为3组：对照组30例,静脉注射0.9%氯化钠注射液3 m L;乌司他丁10万U组30例,静脉注射乌司他丁10万U/3 m L;乌司他丁20万U组30例,静脉注射乌司他丁20万U/3 m L。2 min后松开止血带,3组患者均推注0.06 mg/kg罗库溴铵,并行注射部位疼痛评分。结果对照组、乌司他丁10万U组和20万U组罗库溴铵注射痛总发生率分别66.7%、70.0%、20.0%,3组间比较差异有统计学意义（P〈0.01）,乌司他丁20万U组显著低于对照组、乌司他丁10万U组（P〈0.01）。3组患者均无明显不良反应发生。结论静脉预注乌司他丁10万U和20万U预防罗库溴铵注射痛安全、有效,且以乌司他丁20万U效果更佳。Objective To explore the effects of ulinastatin pretreatment on preventing the injection pain induced by rocuronium. Methods A total of 90 patients underwent general anesthesia admitted to Ya＇ an People＇s Hospital from Jan. 2013 to May 2013 were divided into three treatment groups（ with 30 cases in each group） according to random number table. After occluded venous drainage using a tourniquet on the upper arm,patients in the control group received intravenous injection of 3 m L of 0. 9% sodium chloride solution,patients in the ulinastatin 100 000 U group received intravenous injection of 100 000 U /3 m L of ulinastatin,patients in the ulinastatin 200 000 U group received intravenous injection of 200 000 U /3 m L of ulinastatin.The tourniquet was released after 2 min and then 0. 06 mg / kg of rocuronium was injected within 5 seconds,the pain scales of the injection sites after the injection of rocuronium were evaluated. Results The total incidence rates of injection pain induced by rocuronium of the control group,ulinastatin 100 000 U group,and ulinastatin 200 000 were 66. 7%,70. 0%,20. 0%,the differences were statistically significant（ P 0. 01）,the pain scale of ulinastatin 200 000 U group was significantly lower than that of the control group and ulinastatin 100 000 U group（ P 0. 01）. No adverse reactions occurred in the three groups. Conclusion Intravenous pre-injection of ulinastatin is safe and effective in releasing the injection pain induced by rocuronium,and effect of 200 000 U of ulinastatin is better.
