保留部分腮腺浅叶腺体与全切术在腮腺浅叶良性肿瘤治疗中的临床疗效分析

摘　　要：目的探讨保留部分腮腺浅叶腺体与全切除术在腮腺浅叶良性肿瘤治疗中的应用效果。方法 90例腮腺浅叶良性肿瘤患者被随机分成两组,对照组行传统腮腺浅叶切除术,观察组行保留部分腮腺浅叶腺体切除术,对比两组患者手术情况、术后并发症发生情况及肿瘤复发率。结果观察组的手术时间、术中出血量、切口长度及解剖面神经分支术分别为(83.45±2.13)min、(40.17±3.45)m L、(6.26±1.67)cm和(1.66±0.36)支,均较对照组显著减少,差异具有统计学意义(t=7.32、6.89、7.60、8.37,P<0.05);观察组术后皮下积液或涎瘘、暂时性面瘫、耳垂麻木、面部畸形、Frey综合征等并发症发生率分别为4.44%、11.11%、17.78%、0%和8.89%,均较对照组显著降低,且组间比较差异具有统计学意义(χ2=8.54、6.89、8.93、7.69、7.15,P<0.05);两组患者术后1年肿瘤复发率比较,差异无统计学意义(χ2=0.32,P>0.05)。结论保留部分腮腺浅叶腺体切除术治疗腮腺浅叶良性肿瘤临床疗效较好,可缩短手术时间,减少手术创伤,降低术后并发症的发生,有利于术后恢复,可进行临床推广。Objective To investigate the clinical effect of keeping a portion of the superficial parotid gland and hysterectomy in treatment of superficial lobe of parotid gland benign tumor. Methods A total of 90 cases of patients with superficial parotid gland benign tumor were randomly divided into two groups, the control group received traditional superficial parotidectomy, while the observation group used the retention of some superficial lobe of parotid gland resection. The operation, complications and recurrence rate of the two groups of patients were compared. Results The operative time, bleeding volume, length of incision and dissection of the facial nerve branch operation in the observation group were（83.45 ±2.13） min,（40.17±3.45） m L,（6.26±1.67） cm and（1.66±0.36）, significantly lower than those in the control group（t=7.32, 6.89, 7.60, 8.37, P〈0.05）; The occurrence rate of complication of subcutaneous effusion or salivary fistula, temporary facial paralysis, earlobe numbness, facial deformity, Frey syndrome patients in the observation group were 4.44%, 11.11%, 17.78%, 0% and 8.89%, was significantly lower than the control group, with statistical significance（χ2=8.54, 6.89, 8.93, 7.69, 7.15, P〈0.05）; and the tumor recurrence rates after 1 year of the two group had no significant difference（χ2=0.32, P〉0.05）. Conclusion Keep some superficial parotid resection has a good clinical effect in treatment of superficial parotid gland benign tumor, which can shorten operation time, reduce the surgical trauma and reduce the incidence of postoperative complications, conducive to postoperative recovery, and that can be used in clinical promotion.
