综合性护理干预对腹腔镜高龄前列腺癌根治术患者心理状况及并发症的影响

摘　　要：目的探讨综合性护理干预对腹腔镜高龄前列腺癌根治术患者心理状况及并发症的影响。方法选取2012年6月~2014年6月行腹腔镜前列腺癌根治术68例为研究对象,全部患者根据随机数字表将患者分为干预组及对照组,每组各34例,对照组行随机对症护理,干预组着重实施围术期综合性护理干预,包括术前心理干预及饮食及肠道准备、做好术前各项检查,以及术后一般护理、人工气腹的护理、引流管的护理、饮食护理、膀胱痉挛的护理,比较两组患者干预前后SAS、SDS评分以及两组患者术后并发症情况。结果干预组和对照组患者干预后的SAS评分、SDS评分较干预前显著降低,且干预组患者干预后的SAS评分、SDS评分均显著低于对照组（P〈0.05）。干预组术后无一例出现吻合口狭窄,并发症发生率为8.82%,显著低于对照组的32.40%（P〈0.05）。结论综合性护理干预可以显著改善腹腔镜高龄前列腺癌根治术患者的负性心理,降低术后并发症的发生率。Objective To investigate the influence of comprehensive nursing intervention on psychological status and complications in elderly patients undergoing radical laparoscopic prostate cancer. Methods From June 2012 to June2014, laparoscopic radical prostatectomy in 68 cases as the research object, according to the random number table were divided into intervention group and control group, 34 cases in each group, the control group was treated with random and symptomatic care, the intervention group focused on the implementation of perioperative comprehensive nursing intervention, included preoperative psychological intervention and dietary and intestinal preparation, the examination before operation, and postoperative nursing, artificial pneumoperitoneum nursing, drainage tube nursing, diet nursing, nursing of bladder spasm, before and after the intervention SAS, SDS score and two groups of patients with postoperative complications were compared. Results The SAS score and SDS score of intervention group and control group after the intervention significantly lowered than before intervention, and the SAS score and SDS score of intervention group significantly lower than those in the control group（P〈0.05）. No case of incision stenosis after operation, the complication rate was 8.82% in intervention group, the complication rate was 32.40% in the control group,intervention group was significantly lower than the control group（P〈0.05）. Conclusion The comprehensive nursing intervention can significantly improve the laparoscopic radical prostatectomy negative psychology in elderly patients, reduce the incidence of complications.
