LC术治疗合并血吸虫性肝硬化的胆囊结石42例体会

摘　　要：目的通过采用LC术治疗合并血吸虫性肝硬化的胆囊结石观察其治疗效果以及与传统手术的区别。方法患者42例,根据肝功能Child分级,A级32例,B级10例,全麻插管后,采用三孔或四孔法,行LC手术,术后观察患者恢复情况。结果术后42例均成功实施LC术,术后12例放置腹腔引流管,5例出现一过性腹水,2例出现一过性转氨酶升高。结论血吸虫性肝硬化合并胆囊结石行LC首先把握好手术指征,其次把握好分离技巧,对胆囊三角区胆囊床曲张血管的恰当处理,对于腔镜手术有经验者是相对安全的。Objective To observe its medical results and the differences between this way and the traditional one by using LC operation to treat cholecystolithiasis of schistosomal cirrhosis. Method 42 cases were divided into two groups, 32cases of Level A and 10 cases of Level B, according to Child classification of liver function. LC operations were performed under general anesthesia by using either three-port or four-port technique. And observe the postoperative recovery of the cases. Results LC operations were successful on the 42 cases. 12 cases placed with the abdominal drainage, 5 cases with ascites and 2 cases with transient transaminase lifts. Conclusion When you treat cholecystolithiasis of schistosomal cirrhosis by LC, you should grasp surgical indications first, then the separation techniques and proper processing of gallbladder triangle gallbladder bed varices and blood vessels as well. It is relatively safe for the experienced surgeons to do so under endoscopy operation.
