摘  要:目的:探讨中重型创伤性脑损伤患者早期血清p NF-H、AQP4水平变化及临床意义。方法:选取中重型创伤性脑损伤患者30例(观察组)及健康体检者15例(对照组),应用ELISA法检测患者伤后24 h内及伤后48、72、120 h血清中p NF-H、AQP4水平,分析其变化趋势与疾病演变和预后的关系。结果:观察组患者各时间点血清p NF-H、AQP4浓度较对照组显著高,差异有统计学意义(P<0.05),血清AQP4高峰出现于伤后72 h。重型颅脑损伤患者血清p NF-H、AQP4浓度明显高于中型患者。预后恶劣组患者血清AQP4、p NF-H浓度显著高于预后良好组(P<0.05)。结论:中重型创伤性脑损伤早期患者血清AQP4及p NF-H水平测定有助于评估的病情严重程度和预后。Objective:To explore the change and clinical significance of serum Aquaporin-4(AQP4) and phosphorylated axonal form of the high molecular weight neurofilament subunit NF-H(pNF-H) in moderate and severe traumatic brain injury(TBI) patients.Method:A total of 30 patients with moderate and severe TBI(the observation group) and 15 healthy controls(the control group) were enrolled in this study.Serum samples were collected within 24 h,and 48,72 and 120 h post injury.Serum AQP4 and pNF-H were estimated using ELISA to analyze the change trend of them together. Result:Serum levels of AQP4 and pNF-H in the observation group were significantly higher than those in the control group in all the time(P〈0.05).There was a significant elevation in the levels of the AQP4 in 72 h post injury.The serum AQP4 and pNF-H in patients with severe TBI were higher than those in patients with moderate TBI.The serum levels of AQP4 and pNF-H were significantly higher in patients with poorer prognoses compared with those with better prognoses(P〈0.05).Conclusion:Determination of serum AQP4 and pNF-H in TBI during early period may be useful to evaluate pathogenic condition and prognosis.
