PIK3CA蛋白表达异常与宫颈病变的关系

摘　　要：目的：探讨PIK3CA蛋白表达与宫颈上皮瘤变、宫颈癌的关系。方法：选择2001至2012年在我院治疗的宫颈上皮内瘤变（cervical intraepithelial neoplasia,CIN）90例（CINⅠ,Ⅱ,Ⅲ各30例）,年龄22-61岁,平均年龄（49±5.2）岁;宫颈鳞癌60例（Ⅰ,Ⅱ,Ⅲ期各20例）,年龄28-84岁,平均年龄（60±2.1）岁;另选择因卵巢病变切除子宫的10例正常宫颈组织作为对照组,年龄21-65岁,平均年龄（54±3.3）岁。术前所有患者均未接受化学、免疫及放射治疗。本研究应用原位杂交来检测PIK3CA mRNA表达阳性率,进一步判断PIK3CA mRNA的表达与宫颈癌组织学分化程度、临床分期及淋巴结转移等参数之间关系。结果：PIK3CA mRNA在宫颈癌组与CINⅢ组之间比较,及CINⅡ/CINⅠ组和正常宫颈上皮组间比较差异均有统计学意义（P〈0.01）;PIK3CA mRNA的表达在宫颈癌分期及组织学分化程度中各组间比较差异均有统计学意义（P〈0.01）;PIK3CA mRNA的表达在有无淋巴结转移组间比较差异无统计学意义（P〉0.05）。结论：宫颈病变的恶性演变过程与PIK3CA mRNA的表达具有相关性。Objective： To discuss the relation of the PIK3 CA protein expression with cervical intraepithelial neoplasia（ CIN） and cervical cancer. Methods： From 2001 to 2012,90 cases of CIN（ CINⅠ,Ⅱ,Ⅲ,30 cases each）,22-61 years old,median age（ 49 ± 5. 2） years old,60 cases of cervical squamous cell cancer（ Ⅰ,Ⅱ,Ⅲ,20 cases each）,28-84,average age（ 60 ± 2. 1）years were selected. Replacement for ovarian lesion excision of normal uterine cervix in 10 cases was selected as the control group,21-65,average age（ 54 ± 3. 3） years old. All patients were not receiving any treatment,including chemistry,immunology and radiation therapy. In situ hybridization was used to judge the positive expression rate of PIK3 CA mRNA. PIK3 CA mRNA of expression and degree of histological differentiation,clinical staging of cervical carcinoma and lymph node metastasis and other parameters were further judged. Results： Positive rate of expression of PIK3 CA mRNA in cervical cancer group and CIN Ⅲ group,in CIN Ⅱ / CINⅠ group and normal cervical epithelium had significant difference（ P〈0. 01）. PIK3 CA mRNA expression in cervical cancer staging and histological differentiation degree of each group was statistically significant（ P〈0. 01）. PIK3 CA mRNA expression in with and without lymph node metastasis had no statistical significance（ P〈0. 05）. Conclusion： The expression of PIK3 CA mRNA can diagnose the malignant degree of cervical lesions.
