摘 要:目的探讨不同分子亚型年轻乳腺癌的临床及病理特点。方法回顾性分析新疆医科大学附属肿瘤医院乳腺外科及新疆医科大学第二附属医院肿瘤外科2012年1月—2014年4月收治的146例均经术后病理证实的浸润性乳腺癌患者的临床资料(年龄均≤40岁),按照患者乳腺癌组织ER(雌激素受体),PR(孕激素受体),HER-2(人表皮生长因子受体2),Ki-67(肿瘤细胞增殖相关核抗原)的表达状况分为Luminal A型、Luminal B型、HER2过表达型、Basal-like型4种分子亚型,并统计分析各亚型在临床病理特征方面的差异。结果 Luminal A型24例;Luminal B型77例;HER2过表达型17例;Basal-like型28例;各分子亚型在肿块大小及组织学分级方面的差异有统计学意义(P<0.05);在病理类型,淋巴结转移和临床分期方面的差异无统计学意义(P>0.05)。结论不同分子亚型年轻乳腺癌在相关临床病理特征方面存在差异,这预示着不同分子亚型在治疗及预后方面有着不同的特点,为开展乳腺癌个体化治疗提了供理论依据。Objective To study the clinico-pathologic characteristics of different molecular subtypes of young breast cancer patients.Methods The clinical data of 146 breast cancer patients( ≤40 years old) treated in Department of Breast Surgery,The Cancer Hospital Affiliated to Xinjiang Medical University and Department of Oncology,Second Affiliated Hospital of Xinjiang Medical University,from January 2012 to April 2014 were retrospectively analyzed. These 146 patients,all diagnosed as invasive cancer,were categorized into four molecular subtypes: Luminal A,Luminal B,HER2 over-expression and Basal-like,based on the expression of ER,PR,HER2 and Ki-67. Then the differences in clinical pathological characteristics among different molecular subtypes were analyzed. Results There were 24 patients in Luminal A subtype,77 in Luminal B subtype,17 in HER2 over-expression subtype,and 28 in Basal-like subtype.There were statistically significant differences among different molecular subtypes regarding the tumor size and histological grade,however,there were no statistically differences regarding the pathological type,lymph node metastasis and clinical stage. Conclusions Among different molecular subtypes of young breast cancer there are statistically significant differences in some clinical pathological characteristics,which indicate that different molecular subtypes may have different characteristics in treatment and prognosis. Therefore,it will provide a theoretical basis for individual treatment in breast cancer.
