以皮肤损害为首发表现的慢性粒单核细胞白血病转为急性单核细胞白血病1例

摘　　要：患者男,64岁。2009年3月前因胸痛伴发热、血常规异常4周余就诊,根据临床表现及实验室检查确诊为慢性粒单核细胞白血病(CMML),经化疗后好转出院。出院后1月余躯干、四肢出现红色丘疹、结节、浸润性斑块,组织病理为皮肤白血病。骨髓检查、免疫分型证实为急性单核细胞白血病(AMLM5)。A 64-year-old male with acute myeloid leukemia M5 type （AML-M5） first presented by cutaneous lesion transformed from chronic myelomonocytic Leukemia （CMML） is reported. The patient had chest pain and fe- ver in March 2009. According to the clinical and laboratory findings, a diagnosis of chronic myelomonocytic Leukemia （CMML） was made. After chemo-treatment, the patient took a better turn and left hospital. But about one month latter, red papule, nodules and plaques on body were found. The histopathologic feature of lesion was characteristic of leukemia cutis. The bone marrow and immune monoclonal antibody examination revealed acute myeloid leukemia M5 type （AML-M5）.
