重症肌无力患者胸腺组织抗原处理与提呈相关基因表达的研究

摘　　要：目的探讨抗原处理和抗原提呈在重症肌无力患者胸腺组织T细胞异常分化中的作用。方法收集5例重症肌无力(myasthenia gravis,MG)患者(MG组)和5例非自身免疫性先天性心脏病患者(对照组)胸腺组织,提取RNA,反转录为cDNA,采用树突状细胞与抗原提呈相关基因芯片进行实时定量PCR,采用ΔΔCt方法对数据进行分析,比较2组ΔCt值。结果 2组树突状细胞和抗原提呈细胞芯片中含有84种共8类基因;MG患者胸腺组织表达水平明显增高基因有9种,包括与抗原摄取、提呈相关的TAP2、CD4、THBS1和与细胞因子及细胞因子受体相关的CCL2、CCL3、CXCL2、干扰素-γ(interferon-γ,IFN-γ)、白细胞介素-6(interleukin-6,IL-6)、ERBB2;MG组CCL2(-0.34±0.06),CCL3(3.03±0.45),CD4(-2.95±0.11),CXCL2(0.66±0.21),ERBB2(3.25±0.39),IFN-γ(3.74±0.44),IL-6(1.64±0.22),TAP2(-1.51±0.15),THBS1(0.76±0.55)基因ΔCt值与对照组(3.64±0.03、4.11±0.12、-0.56±0.05、2.26±0.86、4.53±0.48、6.37±0.87、5.76±1.06、0.05±0.01、3.83±0.76)比较差异均有统计学意义(P<0.05);9种基因MG组2-ΔΔCt/对照组2-ΔΔCt均>2。结论 MG患者胸腺组织多种抗原提呈作用相关的基因表达存在异常,可能与T细胞在胸腺的异常分化有关。Objective To study the effect of antigen processing and presentation on abnormal polarization of T cells in thymus tissue in patients with mysthenia gravis（MG）.Methods The total RNA was extracted from thymus tissue in 5MG patients（MG group）and 5patients with non-autoimmune disease（control group）,and finished first strand cDNA synthesis reaction.Real-time PCR detection was performed in human dendritic and antigen presenting cell PCR array.The data were analyzed by usingΔΔCt method andΔCt values were compared between two groups.Results The human dendritic and antigen presenting cell PCR array profiled the expression of 84 genes in 8classes in 2groups,in which 9genes increased significantly higher in thymus tissue in MG patients,such as antigen uptake-and presentation-related TAP2,CD4 and THBS1,and cytokines and cytokine receptors-related CCL2,CCL3,CXCL2,interferon-γ（IFN-γ）,interleukin-6（IL-6）and ERBB2.TheΔCt values of CCL2（-0.34±0.06）,CCL3（3.03±0.45）,CD4（-2.95±0.11）,CXCL2（0.66±0.21）,ERBB2（3.25±0.39）,IFN-γ（3.74±0.44）,IL-6（1.64±0.22）,TAP2（-1.51±0.15）and THBS1（0.76±0.55）in MG group were significantly different from those in control group（3.64±0.03,4.11±0.12,-0.56±0.05,2.26±0.86,4.53±0.48,6.37±0.87,5.76±1.06,0.05±0.01,3.83±0.76）（P〈0.05）.The ratio of 2-ΔΔCt ratio of 9genes in MG group to control group was larger than 2.Conclusion The gene expressions are abnormal in antigen processing and antigen presenting in thymic tissue in MG patients,which may be correlated with abnormal differentiation of T cell.
