浸润性乳腺癌、乳腺良性病变及正常乳腺组织中骨桥蛋白及相关因子的表达差异及相关性研究  被引量:6

Differential expression of osteopontin and related factors in invasive breast cancer, benign breast lesion,normal breast tissues and correlation analysis

摘  要:目的检测不同性质乳腺组织中骨桥蛋白(OPN)、血管内皮生长因子-C(VEGF-C)和乳腺肿瘤激酶(Brk)表达量的差异并探讨其临床意义。方法采用免疫组织化学染色法检测92例浸润性乳腺癌组织、60例乳腺良性病变组织和35例正常乳腺组织中OPN、VEGF-C和Brk的表达情况,比较分析其与临床病理学因素之间的相关性。结果三种蛋白OPN、VEGF-C和Brk在浸润性乳腺癌组织中均呈现出较高的表达,阳性表达率分别为66.3%、56.5%和70.7%,与乳腺良性病变以及正常乳腺组织比较差异均具有显著统计学意义(P<0.01)。OPN、Brk表达与患者年龄、肿瘤直径以及组织病理学分级无关(P>0.05),而与淋巴结转移及TNM分期有关(P<0.05);而VEGF-C表达与患者年龄、肿瘤直径无关(P>0.05),与组织病理学分级、淋巴结转移及TNM分期有关(P<0.05)。乳腺癌组织中的OPN、VEGF-C和Brk表达两两之间呈正相关性(P<0.05),而在正常乳腺组织和良性病变组织中三者之间无明显相关性(P>0.05)。结论在正常乳腺组织、良性病变组织和乳腺癌组织中,OPN、Brk与VEGF-C的表达量均依次升高。OPN可能通过介导VEGF-C蛋白刺激内皮细胞黏附和迁移而加速血管形成,通过参与新血管生成和肿瘤的侵袭进而与淋巴结转移有关,三者的表达量均与肿瘤分期、淋巴结转移等临床病理因素有不同程度的相关。Objective To detect the differential expression of osteopontin(OPN), vascular endothelial growth factor C(VEGF-C) and breast tumor kinase(Brk) in different breast tissue types, and to evaluate its clinical significance. Methods Immunohistochemical staining was applied to detect the expression of OPN, VEGF-C and Brk in 92 cases of invasive breast cancer, 60 cases of benign breast lesions and 35 cases of normal breast tissue.Their correlation with clinicopathological factors were compared and analyzed. Results OPN, VEGF-C and Brk proteins in invasive breast cancer all showed high positive expression rates of 66.3%, 56.5% and 70.7%, respectively,which had statistically significant difference with those in benign breast lesions and normal breast tissue(P〈0.01).The expressions of OPN, Brk showed no correlation with age, tumor size and histological grade(P〉0.05), but were correlated with lymph node metastasis and TNM stage(P〈0.05). VEGF-C expression were correlated to histopathologic grade, lymph node metastasis and TNM stage(P〈0.05), but not with age, tumor size(P〉0.05). In breast cancer,there was positive correlation between the expression of OPN, VEGF-C and Brk(P〈0.05), no with no significant correlation in normal breast tissue and benign breast lesions(P〉0.05). Conclusion In normal breast tissue, benign breast lesions and breast cancer, the expression levels of OPN, Brk and VEGF-C increase successively. OPN,through mediating VEGF-C, may stimulate endothelial cell adhesion and migration to accelerate angiogenesis. It is involved in angiogenesis and tumor invasion and thus correlated with lymph node metastasis. The expression levels of OPN, Brk and VEGF-C are correlated to clinical pathological factors of tumor stage, lymph node metastasis to different extents.
