Hp感染、p53、ASPP2、iASPP表达与胃癌及癌前病变关系的临床研究

摘　　要：目的探讨幽门螺杆菌（Hp）感染在胃癌（GC）发生中与p53、ASPP2、iASPP基因表达的关系。方法收集2011年5月-2013年4月聊城市人民医院140例疑似GC患者的胃黏膜标本,男性83例,女性57例,年龄38-69岁,平均52.5岁。病理学检查结果证实,胃部良性病变（BGD）组29例,胃癌前病变（pre-Ca）组47例,GC组64例。采用Warthin-Starry染色法和快速尿素酶试验观察不同胃病黏膜组织中Hp感染情况,采用免疫组化S-P法检测p53、ASPP2、i ASPP基因蛋白表达情况。结果 GC组、pre-Ca组Hp感染率明显高于BGD组（P〈0.05）。i ASPP、p53及ASPP2在Hp阳性pre-Ca组和Hp阳性GC组中的阳性表达率均高于Hp阳性BGD组（P〈0.05）。i ASPP、p53在Hp阳性GC组中的阳性表达率高于Hp阳性pre-Ca组（P〈0.05）。结论 Hp与GC的发生密切相关,在GC的发生、发展过程中,Hp感染与细胞凋亡调控蛋白p53、ASPP2、i ASPP可能存在相互协同关系。Objective To explore the relationship between Helicobacter pylori（Hp）and genetic expression of p53,ASPP2 and iASPP in gastric carcinoma. Methods Specimens of gastric mucosa from 140 patients with suspected gastric carci- noma in our hospital from May 2011 to April 2013 were collected.Among them,there were 83 cases in male and 57 cas- es in female.Their ages were ranged from 38 to 69 years old,at an average of 52.5 years old. Result from pathological examination showed benign gastric diseases（BGD） group of 29 cases,precancerous lesion（pre-Ca） group of 47 cases and GC group of 64 cases.By Warthin-Starry staining method and rapid urease test,Hp infection in mucosal tissues from different gastropathy was observed.Gene protein expressions of p53,ASPP2 and iASPP were detected by immunohisto- chemical S-P method. Results Hp infection rate of GC group and pre-Ca group was higher than that of BGD group re- spectively （P〈0.05）.The positive expression rate of iASPP,p53 and ASPP2 in pre-Ca with Hp positive group and GC with Hp positive group was higher than that in BGD with Hp positive group respectively （P〈0.05）.The positive expres- sion rate of iASPP and p53 in GC with Hp positive group was higher than that in pre-Ca with Hp positive group re- spectively （P〈0.05）. Conclusion Hp is closely related with the occurrence of GC.In the occurrence and development of GC,Hp infection has a mutual and synergic relation with apoptosis regulatory proteins including p53,ASPP2 and iASPP.
